OC Re-Inspection - E&D Class 4 This inspection was conducted with the owner. A PDF copy of this inspection report was emailed to Stephanie L. at the time of this inspection. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, or by phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable. Correct the following prior to your first routine inspection; 1. The RPZ backflow prevention devices for the kitchen soda fountain and bar soda fountain must each be tested and certified. 2. The drain pipe for the main ice machine is leaking into a pan beneath the machine then draining into a floor drain. Repair all leaks in the main ice machine drain system. All drain pipes for the ice machine must be hard, PVC type piping (no flex piping) and routed to drain into the floor sink beneath the food prep sink with a minimum 1 in. air-gap. The owner stated that this is scheduled to be repaired in the near future. 3. Provide a minimum 1 in. air-gap between the end of the scrap sink drain line and the flood rim of the floor sink that it drains into beneath the dishwasher. 4. Provide a cover plate for the clean out opening in the wall beneath the mop sink faucet. 5. Finish repairing the broken door on the prep cooler in the main cooking area. 6. Replace all burnt out lights in the kitchen and provide shields or tube shields for the lights. 7. Remove all foam, melted caulking and melted plastic trim from beneath the exhaust hood. Re-caulk the seam between the bottom of the hood with high temperature resistant, metallic, silicone caulk. 8. Repair the cracks and seams in the floor in the main cook line area to be smooth, non-absorbent and easily cleanable. No county legal action will result from this inspection.

Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control.
