Inspection Comments

E&D, class 2 10+ OC inspection was conducted with the permit holder, Denise. The establishment is open and operating. Permit is not approved. Person in charge must be a certified food protection manager and available on site during operational hours. Maintain all managers and food handlers’ certifications on site for inspection purpose. Please address the following items before the re-inspection: 1. Noted fruit fly inside the establishment. Please contact a licensed pest control company to eradicate the pests. 2. Disconnect and remove the blender rinser from the food prep sink. Otherwise, provide a separate water feed with shut-off valve for the blender rinser and provide a reduced pressure zone (RPZ) assembly (ASSE standard 1013) at the water connection point of the blender rinser. 3. Provide 1” air gap minimum from the finished floor for drainpipe from the ice machine and the 3-comp sink. 4. Elevate the drainpipe from the ice machine to 6 inches above the floor, slope the drainpipe to the nearest floor sink. Drainpipes must be at least 6” above the floor. 5. Provide a faucet for the dipper well. A hose feeding the faucet is not acceptable. A water saving hot water dipper well can be used for this purpose also. 6. Re-attach the insulation panel of the coffin type freezer to the lid. It is falling off. 7. Elevate the water lines for the dipper well off the floor. Water lines must be 6” or more off the floor. 8. Provide additional shelving, dunnage racks, cabinets for single service/ single used items, merchandises, condiments, etc. so these items are off the floor. All items must be stored 6” off the floor. 9. Remove the un-used flexible drainpipe under the smoothie prep area. No County legal action will result from this inspection. The inspection rights sheet is emailed with the report to the manager. Please contact the Department at 602-506-6824 to schedule the next inspection. Allow 7-15 business days to schedule. You are hereby notified that you are operating this establishment in violation of the MCEHC. Per Chapter 1, Regulation 4(a) “No person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so, unless otherwise specified in this Environmental Health Code.” Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections.

Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
