Retail food, class 2 OC re-inspection was conducted with the permit holder, Zaman. Construction is at 90% completion. Owner would like to sell pre-packaged non-temperature control food and soda fountain drink. The establishment can do so at this time. Coffee/ tea machines will be installed in a few weeks. Permit is approved. Please address the following items prior to the next inspection: 1. Maintain cold holding equipment at 36-38*F. Walk-in cooler was holding at 49 and 51*F respectively. There is no time/ temperature control food in the facility at the time. Repair and call for verification visit. 2. Provide a manifold for the drainpipes from drip trays or drainpipes of the juice/ coffee equipment. Brace along the wall and slope to drain to the nearest floor sink with minimum 1” air gap from the finished floor. 3. Provide test report for the backflow preventer of the soda carbonator. 4. Provide an ASSE 1022, vented dual check valve, at the water connection point of each coffee/ tea brewer or juice machine. 5. All backflow preventers must be installed in a visible and easily accessible location for monitoring and servicing. 6. Provide side splashguard on the food prep sink if any work surface or equipment station within 24 inches of the sink. Splashguard must be at the height of the faucet and length of the sink. The report was e-mailed to the owner. No county legal action will result from this inspection. You are hereby notified that you are operating this establishment in violation of the MCEHC. Per Chapter 1, Regulation 4(a) “No person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so, unless otherwise specified in this Environmental Health Code. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.
