Inspection Comments

OC inspection for a Class 5 Food Catering permit was conducted with manager Reyna V. by appointment. Inspection rights were delivered via email. The establishment was closed, but is operational. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please address the following items prior to the OC re-inspection within 30 days: 1) Install an ASSE 1013 reduced pressure backflow preventer upstream of the mister pump and misting system. Certify and provide a test report. 2) Install a hub drain at the outdoor water fill station and provide a 1" minimum air gap for the water fill drain line. Remove the flex drain line and provide rigid piping. 3) Provide 1" minimum air gaps throughout the bar indirect drain lines. Transition the drink rail flexible drain to a rigid piped drain and air gap. 4) Provide test reports for all RP backflow preventers installed throughout the facility. 5) Provide air curtains to the garage roll-up doors and windows or provide a completed IPM/AMC program. Information provided via email. 6) Caulk-in all escutcheon plates and plumbing penetrations throughout the facility with silicone. 7) Re-caulk the sink fixtures and drain boards throughout the facility with silicone. 8) Caulk the base of the employee restroom toilet. 9) Clean the ventilation fan in the employee restroom and the surrounding walls. 10) Re-caulk the ventilation hood gaps with a high temperature, aluminum silicone. 11) Re-position the right prep sink drain to prevent the oversplash on the ground. 12) Re-grout the floor tile areas at the main bar to provide a smooth and flush finish. 13) Silicone all trimmed foam sealant throughout the facility. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
