E&D, class 3 10+ OC inspection was conducted with the manager, Amy B. The establishment opens and operating. Manager is not a certified food protection manager. Please obtain and maintain all managers and food handlers’ certifications on site for inspection purpose. Permit is approved. Please address the following items prior to the re-inspection on 6/21/22: 1. Repair/ replace the reach-in refrigerator under the coffee brewer, make table with cream/ milk and juice/ butter merchandiser so they can maintain cold time/ temperature control foods for public health safety at 41*F or below. Currently they have the ambient air temperature at 80*F, 54*F and 64*F respectively. Milk and cream containers were relocated to another working refrigerator, butter/ sour cream and nut creams in portioned cups were discarded. Please send pictures/ video of the equipment after they were repaired. 2. Provide 1” air gap from the finished floor for the drainpipe of the ice machine. 3. Adjust the drainpipe from the vent port of the 1022 backflow preventer so it is facing down and slope to drain into the floor sink with minimum 1” air gap from the finished floor. 4. Relocate the flat grill at the front service area so the equipment is 6 inches inside the hood. It is currently sitting at the edge of the hood and grease laden smoke is not contained. 5. Clean all hood baffles. Noted heavy grease built up in the baffles at the hood near the back door. 6. Replace burnt/ dimmed light where needed. 7. Provide additional lighting inside the mop sink area to reach 20-foot candlelight. Currently it is at 10-foot candlelight. 8. Replace missing/ cracked/ broken floor tiles throughout. 9. Provide light and washable ceiling tiles areas behind the counter, kitchen, restroom and mop sink area. No County legal action will result from this inspection. The inspection rights sheet is emailed with the report to the person in charge. Please contact the Department at 602-506-6824 to schedule the next inspection. Allow 7-15 business days to schedule. You are hereby notified that you are operating this establishment in violation of the MCEHC. Per Chapter 1, Regulation 4(a) “No person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so, unless otherwise specified in this Environmental Health Code.” Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.

Priority foundation violation is a minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation.
e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
