OC Inspection, E&D Class 4, 10+ seating. Inspection conducted with owner Marvin T. Establishment was closed and not operating at the time of inspection. Inspection report and rights emailed to Marvin T. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Correct the following items prior to reinspection on 6/7/2022: 1) Provide an NSF/ANSI approved prep sink with hot and cold water. Drain line must be indirectly drained to a floor sink with a 1-inch air gap. 2) Replace the two flex drain lines near soda equipment for rigid drain lines. 3) Provide a 1-inch air gap at the floor sink for all indirect drain lines. Ensure all drain lines slope to drain and aren’t sitting on the floor. 4) Relocate the RPZ backflow device behind the soda equipment. The RPZ backflow device is not easily accessible. 5) Test and certify the RPZ backflow device and provide the test report. 6) Replace the residential fridge at the front of the restaurant with a commercial approved NSF/ANSI fridge. 7) Provide high temperature silver colored silicone to all the gaps of the hood. Remove the old silicone first. 8) Replace the rusted shelving in the walk-in cooler. 9) Repair the right-side panel of the walk-in cooler. Observed rust on this side of the walk-in cooler. 10) Deep clean underneath the wok/cook line area, ceiling tiles, and walk-in cooler. 11) Replace the water damaged and 1 perforated ceiling tile in the kitchen as discussed. Ensure ceiling tile is washable vinyl. 12) Provide a stainless-steel cover to the open pass through window. Or remove the picture covering the pass-through window. 13) Add weather stripping to the left side of the back door. Observed light shining through. 14) Remove all maintenance tools from the kitchen. 15) Provide grates for all floor sinks. 16) Provide a clean out cover for the women’s restroom. Please contact the Department to schedule a follow-up inspection at 602.506.6980 once violations have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. No County legal action will result from this inspection.

Priority foundation violation is a minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation.
e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
