Inspection Comments

Final construction inspection was conducted with the superintendent, Brice, general manager, Teresa N. and Chef, Eddie G. Construction has been completed. Reviewed IMP/ AMC plan at the site. The establishment is approved to follow the outer opening exemption guidance documents. If the facility fails to control pests’ presence inside the facility the outer exemption shall be removed and air curtains/ screens, or doors shall be provided at all outer opening. Food operating permit is approved. Currently the dog friendly patio is not approved. Noted a smoker is chained to the light post outside of the facility. Per chef, the establishment will not use the smoker. Please remove the equipment if not use. An outdoor food operation application is required prior to conducting outdoor food service. Provide guidance document for outdoor food. Please address the following items prior to the next inspection: 1. Provide a vented dual check valve (ASSE 1022) at the water connection point of the coffee/ tea brewer and the espresso machine. The one provided is a dual check valve (ASSE 1024); which is not acceptable. 2. Fill expansion joints on the concrete pads of the dog friendly patio, then seal the floor with several coats of marine grade sealant. Water must bead-up on the floor. 3. Provide DOG FRIENDLY PATIO SIGN at the entrance to the patio area. 4. Provide 1” air gap from the finished floor for drainpipes from soda fountain drink at the drive-through window and the ice bin at the small bar. 5. Ensure all ceiling tiles are pushed in place. 6. Flash off top of the walk-in units to the ceiling. 7. Maintain monitoring logs, check lists and pest control service log on site for inspection purpose. No County legal action will result from this inspection. The report is emailed to owner. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.


Definitions
Priority violation is a major violation that directly contributes to increasing the risk of foodborne illness or injury.
e.g. - Food employees do not properly wash hands when required
Priority foundation violation is a minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation.
e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
