OC inspection conducted with Visitors Service Manager. VSM acknowledged receiving information on inspection rights. Permit approved. Inspection report emailed to Ernesto M. The following items will be addressed on RM-09888. 1. Provide an approved NSF prep sink for the rinsing/thawing of food items. Drain line must be indirectly plumbed with approved air gaps. 2. Provide an above ground floor sink for the drains of the prep sink and ice machine (if installed). 3. The 3-compartment sink may use a riser or HUB drain to accommodate for the floor sink. 4. Provide a hard lid ceiling or grid system with vinyl ceiling tiles above the kitchen area. 5. Provide FRP paneling underneath the 3-comp sink and adjacent hand sink. 6. Seal all the penetrations into the walls (plumbing and electrical) in the kitchen and service area. 7. Seal all gaps, and cracks as needed in the kitchen area. 8. Provide a copy of the certified food manager certification. Of Note: Create an S.O.P. for cleaning the duct work above the kitchen area until hard lid ceiling is installed. Please order precut food items until the prep sink is installed. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. No County legal action will result from this inspection.


Definitions
Priority violation is a major violation that directly contributes to increasing the risk of foodborne illness or injury.
e.g. - Food employees do not properly wash hands when required
Priority foundation violation is a minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation.
e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
