Visited Mirage Market. This store is in the process of being transferred to a new ownership. This store has applied for a retail Grocery Store but was never permitted and permit was not issued. Retail Grocery Class II - A permit to sell pre-packaged foods from an approved source only including TCS foods stored in a commercial refrigerator or freezer. Retail Grocery Class III - A permit to sell pre-packaged foods from an approved source only including TCS foods stored in a commercial refrigerator or freezer with the allowance to break down non-TCS foods into smaller packages. THs requires the installation of a NSF approved three compartment sink and additional hand wash sink and washable finsishes for floors/ walls and ceiling in the area where you break down foods and wash utensils. If you want to proceed you will need to follow up on the items below while the store is still under the current ownership so a permit can be usued; 1). Go online at the state website (search Cottage Law Arizona) and register. 2). Obtain a food handlers card. 3). Remove all the bulk foods from the store. 4). Re-package the food items you would like to break down at home and bring to the store with the proper labelling as directed by the state on their website. Note that only Non-TCS foods may be broken down and re-packaged. No cream, dairy, meats etc. 5). Install washable ceiling tiles over the mop sink. (2). Caulk mop sink at the wall juncture. 6), Make sure all refrigeration equipment is commercial, home style freezers are not allowed. Once that is done and you can present your registration number, show the food handlers card and labelled products on the shelves, bulk products I can issue the permit. Once issued I can close that permit and you will need to apply for your own retail grocery permit. You will need to go online at : https://www.maricopa.gov/FormCenter/Environmental-Services-16/Food-Establishment-Permit-Application-Pe-307 You fill out the application and you will be send an invoice for $315.00. Once paid i will call to schedule the inspection and after inspection issue the permit. This permit will be processed and you will be send an invoice in the mail for the actual permit that you will need to pay in advance for the year. the fee for that permit will be $235.00. This will be a re-occurring annual fee. If the transition to the new ownership occurs before these arrangements above can be made then: 1). The store will be considered new and you will need to submit plans of the store (same online application link as above), menu and spec sheets for the equipment. The fee will be $1250.00 for expedite submittal. 2). You will or need to install the three compartment sink and hand wash sink or follow through on the cottage law guidance above.
