Inspection Comments

New owner inspection walk trough with the regional manager. The following items were identified for follow up: 1). Please check the hand sink water temperature at the cooks line and make sure the water provided is around 100 degrees Fahrenheit. 2). The back room hand wash sink is leaking at the drain, please repair. 3). Re-set the ice machine at the drive through on the SSBS and make sure now water leaks out of the unit at the joint. 3). Level SSBS and see if you can provide cleanable legs. The bricks do not allow for convenient cleaning. 4). Raise or cut back the drain lines on the SSBS unit and provide air gaps om the drains. Flex hoses should be kept short nd be properly braced. 5). At the multiplex, caulk all the holes in the wall and finish out smooth and easily cleanable. 6). Provide a test report for the Reduced Pressure Zone Backflow valve upstream of the Multi-Plex that is less than one year old or have the valve re-tested. 7). Provide the following backflow valves. a). A reduced pressure zone backflow vale upstream of the RO system. b). Dual check valves that meet plumbing standard ASSE 1022 in the following locations: i). Tea Brewer. ii). Coffee Brewers. One may serve both machines. iii). The blended ice machine. 8). Grout is recessed in certain areas, dish wash and ice machine in the back room, please re-grout and provide a flush with the tile grout line. 9). In some areas the base tile is standing loose from the wall or there are gaps. Please seal the top wall joint of the base tile and the FRP. 10). One of the dumpsters is leaking and the lids do not properly close, please replace dumpster and remove soil buildup under the dumpster. 11). At the front mop sink, remove y-splitter and provide a dedicated water line to the eco lab erosion feeder. There are multiple options. 12). In the restrooms, re-caulk/ grout the hand wash counters at the top wall junctures and seal. 13). Under the beverage counter in the dining room, adjust the drain lines and make sure you follow gravity flow to the floor sink. Line drains up so water does not end up on the floor under the toe kick. Clean area. 14). Repair broken base tiles behind the hamburger freezer. 15). Please separate the washer in the back room from the air pocked with the three compartment sink and ice tea brewer. This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. No operating permit is valid, until the permit fee is received by the Department. It is also the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. A re-inspection will be conducted September 29th at 11 AM. Please note that if not all the items have been corrected you may be changed a follow up inspection fee. Permit issued Eating and Drinking Class III 10+ Seating. Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
