OC Inspection - E&D Class 3 This inspection was conducted with the manager on duty. A PDF copy of this inspection report was emailed to Andrew O. at the time of this inspection. The following items must be corrected prior to re-inspection scheduled for 09/13/2022. If violations are not corrected by established timeframes, legal action may result. 1. Both of the waste drain pipes from the trough drains in front of the self-service soda fountains are broken and leaking into the cabinets beneath. The ice machine on top of the right soda fountain is also leaking down the side of the soda fountain and is non-functional. Repair all of these leaks. 2. The drain valve beneath the left compartment of the three compartment sink is leaking onto the floor. Repair this leak. 3. There is an Auto Mist chemical dispenser system connected to a water supply line with no backflow protection installed. Provide a certified, RPZ backflow prevention device to isolate this device from all other equipment and plumbing connected to water supply lines. This system may also be removed to correct this item. 4. There is no backflow protection installed for the water supply line to the Frappe machine that has a blender rinser with a submerged water inlet. Provide a drinking water safe, certified, RPZ backflow prevention device to isolate this machine from all other equipment and plumbing connected to water supply lines. This device must be installed on the water line behind this machine with a funnel drain assembly that empties into a floor sink with a minimum 1 in. air-gap to properly correct this item. 5. The RPZ backflow prevention device for the water supply to the muliti-plex unit must be tested and certified as must the new RPZ device that will be installed for the Frappe machine. Provide the certifications for re-inspection. 6. There is no hot water to the small prep sink where the Ketchup dispenser is located. All sinks must have hot and cold water provided. Provide hot water to this sink. 7. Provide minimum 1 in. air-gaps between all drain pipes in the front beverage area and the flood rims of the floor sinks that they drain into. Some of these pipes currently extend down into the floor sinks. 8. The front hand sink is detaching form the wall. Properly attach and seal the backsplash of the front hand sink to the wall behind it. 9. Provide metal cove base at the floor/wall junctures on the outside of the walk-in refrigerator. This is in the areas to the right and left of the walk-in door. 10, Repair and seal the damaged corners and seams between the kitchen mop sink basin and the surrounding walls. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Priority foundation violation is a minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation.
e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
