Inspection Comments

OC inspection for a Class 3 10+ E&D permit was conducted with owner Pakkawalan P. by appointment. The establishment was open and operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please address the following items prior to the routine inspection: 1) Flip the spring unit for the three sink sprayer so that the sprayer maintains a 1" minimum air gap above the sink flood rim. 2) Remove the curing pink salts from the facility permanently. The use of these salts requires a variance. 3) Remove the self-service garnish tray at the end of the customer line, as discussed. 4) Provide rubber cove to the floor/wall junctures of the wall behind the walk-in cooler and at the back exterior door in the kitchen. 5) Fill all wall holes throughout the facility with silicone. 6) Trim the foam sealant throughout the facility flush and provide a smooth layer of silicone. 7) Caulk-in all escutcheon plates and plumbing penetrations throughout the facility. 8) Seal the small gap between the right side of the walk-in cooler and wall. 9) Provide the new washable, vinyl ceiling tiles above the soda fountain and kitchen areas, as discussed. 10) Refinish or replace the rusted shelving units in the walk-in cooler. 11) Replace the broken light fixtures throughout the facility, as needed. 12) Replace the torn refrigerator door gaskets, as needed. 13) Resurface or replace the heavily scoured and stained cutting boards. The small bar area will require a minor remodel submittal and approval prior to operating the area in the future. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
