Inspection Comments

Final construction inspection for a Class 2 0-9 E&D permit was conducted with Craig V. and Jared S. by appointment. The establishment was open and operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please address the following items prior to the construction inspection within 30 days: 1) Provide an audible alarm for the ice machine condensate pump. 2) Transition the condensate pump drain to rigid PVC piping and provide a 1" minimum air gap above the floor sink. 3) Provide a 1" minimum air gap between the ice machine drain and condensate pump. 4) Provide a stainless backsplash behind the three compartment sink and handwash sink. 5) Provide a clear sealer around the rest of the wood panel wall to provide a smooth, durable, non-absorbent, and easily cleanable surface. 6) Caulk the sink fixtures to the walls with silicone. 7) Repair the detached rubber cove base at the mop sink room and provide additional rubber base at the FRP. 8) Per management, the refrigerators are utilized for staff. Observed nothing in the coolers at time of inspection. Label the coolers as employee use only. 9) Provide a self-closure device to the employee restroom door. 10) Fill the concrete saw cuts at the food service and customer restroom flooring to provide a smooth and flush finish. All food products within 5 feet of customers must be maintained with lids at all times to prevent contamination from consumers. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
