Inspection Comments

E&D, class 4 10+ OC inspection was conducted with the owner, Destiny. The establishment is open and operating. Owner and managers are certified food protection managers. Maintain all managers and food handlers’ certifications on site for inspection purpose. Permit is approved. Please address the following items before your routine inspection: 1. Provide a rigid drainpipe for the drain from the ice well at the drive through area. PVC or ABS is acceptable. Flexible hose is not acceptable. Brace the drainpipe to the equipment and have it indirectly discharged to the floor sink with 1” air gap from the finished floor. 2. Extend the drainpipe from the soda fountain drip tray at the drive-through window so drip liquid can drain into the floor sink below and not at the edge of the floor sink. 3. In lieu of the reduced pressure zone (RPZ) assembly (ASSE 1013) for the soda carbonators, a vented dual check valve (ASSE 1022) was installed upstream of the soda carbonator. These are not testable devices; therefore, they must be monitored. Replace the devices whenever they leak. Another option is to install a reduced pressure zone (RPZ) assembly (ASSE 1013), a testable/ repairable device upstream from all 3 soda carbonators. Provide drain cup for the vent port of the RPZ. Drainpipe must be rigid and indirectly discharge to a floor sink with minimum 1” air gap from the finished floor. Certify the device and provide report. 4. Re-attach the access door for the trap primer below the hand sink near the fryers. 5. Provide missing plugs inside the walk-in units. Please use low temp silicone to help keep the plugs in place. 6. Replace burnt light inside the walk-in freezer. 7. The cold prep table at the burger station’s ambient air temperature was measured at 41-42*F at the time of inspection. Please repair/ adjust to maintain ambient air temp at 37-38*F in order for the products inside the units at 41*F or below. Some items inside the bottom part of the equipment are holding at 42-43*F. 8. Remove old caulking behind sinks, fixed equipment, or drop-in equipment and provide new as needed. 9. Provide thermal test strips, min/ max thermometer or high temp dishwasher thermometer for the high-temp dishwasher. Emailed the report to the owner. No County legal action will result from this inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.

Priority foundation violation is a minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation.
e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
