Store # 7672__E&D Class 2 0-9 OC inspection was conducted with the person in charge, Alec W. The establishment is open and operating. Permit is not approved. Manager is a certified food protection manager. Person in charge must be a certified food protection manager and available on site at all times. Maintain all managers and food handlers’ certification and licensing on site for inspection purpose. Address the following prior to the re-inspection: 1. Provide an additional hand sink at the 3-comp sink’s side. Sink must have hot and cold water and side splashguards if it is within 24 inches of sinks, or prep areas. 2. Provide a pot fill faucet at the prep table / workstation or a food prep sink so the employees can use to add water to the pizza sauce. 3. Provide side splashguard on the right side of the front hand sink. Splashguard must be at the height of the faucet and length of the sink. 4. Repair/ replace the hot water faucet handle of the mop sink. It is leaking profusely when turn on. 5. Repair the drainpipe under the sanitizer sink. It is leaking at the time of inspection. 6. Provide the self-closer for the restroom’s door. 7. Provide missing cove base and threshold tiles on the wall and floor along the back door. 8. Caulk and seal any gaps, cracks and penetrations throughout. This includes holes around electrical conduit, holes, and fire sprinkler inside the walk-in unit. 9. Replace broken / cracked/chipped floor tiles where needed. 10. Remove the fuzzy material around the edge of the hood to allow smooth, non-absorbent, easily cleanable surface. 11. Provide split toilet seat for the toilet. 12. Repair the damaged wall at the manager’s table. No county legal action will result from this inspection. You are hereby notified that you are operating this establishment in violation of the MCEHC. Per Chapter 1, Regulation 4(a) “No person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so, unless otherwise specified in this Environmental Health Code.” Please contact the Department to schedule a follow-up inspection at 602.506.6980 once violations have been corrected.
