OC Inspection. Walked with Ron. E&D Class 4 10+. Correct the following items within 30 days: 1) Provide 3 units of tankless water heater Vesta VRP-199. Only one unit is currently installed. Make/model makes about 6.5 gallons per minute (GPM) at a 60*F rise; current peak fixture demand is 17 GPM. 2) Install a hand sink in warewashing area. Hand sink must have hot and cold water and drain to sewer. 3) Install aerators on all hand sinks and dump sinks, to meet hot water calculation demands (not having aerators installed increases calculated GPM of hand sinks). 4) Provide hot and cold water to hand sink on cookline. Ensure hand sink drains to floor sink with a 1” air gap. 5) Repair/adjust drain line of hand sink in server area so drain line does not leak water onto floor. 6) Hard pipe drain line from beer cooler to floor sink, sloped to drain, with a 1” air gap at floor sink. 7) Hard pipe all drain lines in center prep island in kitchen; flex tubing is not approved. 8) Ensure all hand sink and dump sink in bar have splash guards where necessary (pending final placement of bar equipment). 9) Install FRP on walls where missing such as near cookline, prep area 10) Replace all clean out covers and electrical/light switch covers where missing. 11) Seal all sinks/stationary tables to walls with caulk or silicone. 12) Seal all gaps/cracks/penetrations/holes in walls as discussed on site. 13) Detail clean establishment, including outdoor areas. A sneeze guard is not required at this time, as long as pizza pass-thru is always monitored by an employee. If violations are written on routine inspections for protection of open food in this area, a sneeze guard may be required in the future. If a misting system is installed, an RPZ backflow preventer (ASSE 1013) will be required on water line to misting system. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. No County legal action will result from this inspection. Report emailed to Ron.

e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
