Inspection Comments

OC Inspection. Walked with Aijaz. E&D Class 2 10+ seating. Correct the following items within 30 days: 1) Install an ASSE 1015 backflow preventer (testable in line double check assembly, such as a Watts 007) on water line to water-cooled frozen yogurt machine. Otherwise, disconnect line from machine until machine will be put into use. 2) Provide an indirect connection for drain line from water-cooled frozen yogurt machine, such as by installing a hub drain at front service hand sink. Otherwise, disconnect drain line from machine until machine will be put into use. 3) Keep an approved sanitizer for properly sanitizing food contact surfaces such as chlorine or quaternary ammonia at all times. PIC went to store and returned with chlorine during inspection. 4) Install a slash guard on right side of hand sink in front service area. 5) Stock back hand sink with paper towel. 6) Provide test strips for chloring sanitizer. 7) A Certified Food Manager (CFM) must be on site during all hours of operation. Have employees obtain a CFM as necessary to achieve coverage. 8) Detail clean all refrigerators and freezers. 9) Remove magnets from self-serve condiment containers so that lids are self-closing. 10) Repair/replace lights in back room above 3-compartment sink. 11) Seal all small holes in walls with caulk or silicone. Discussed proper hand sink usagage, hand washing, and how to wash/rinse/sanitize dishes during inspection. PIC states that establishment will be adding more machines to front service area. A remodel permit with MCESD will be required – apply at least 30-45 days before construction to ensure timely review of your plans and application. Any newly installed water cooled machines will need an ASSE 1015 on water line and an indirect connection to sewer as noted above. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Priority foundation violation is a minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation.
e.g.- Hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required)
Core violation is a minor violation that relates to general maintenance and sanitation.
e.g. -No sign reminding employees to wash hands
Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control. Verification Visit inspections found in compliance with Cutting Edge program requirements receive an “A” grade.
