Dear Permit Holder: An inspector for the Maricopa County Environmental Services Department (MCESD) has been unable to access your Mobile Food Establishment, to conduct an inspection. The inspector has also made attempts to contact you by phone and/or email to determine when and where the Mobile Food Establishment will be in operation so they may conduct an inspection. At this time, you have failed to return their phone calls or respond to their emails and have not allowed them access to inspect fully your Mobile Food Establishment. Please be advised that Chapter I of the Maricopa County Environmental Health Code states: REGULATION 3. Responsibilities – Right of Inspection b. No person shall refuse to allow the Department to inspect fully any and all premises or facilities at any reasonable time, and no person shall molest or resist the Department in the discharge of its duties whether in inspections for application approval or for the enforcement of this Environmental Health Code. REGULATION 7. Suspension and Revocation of Permits b. Revocation of Permit: (1) The Environmental Health Officer may, after providing opportunity for hearing, revoke a permit for serious or repeated violations of any of the requirements of this Code or for interference with the regulatory authority in the performance of duty. Failure to provide MCESD with the information for operating days and times as well as allowing a representative of MCESD access to the Mobile Food Establishment to conduct an inspection may result in legal action being taken against the establishment’s operating permit. Please contact the Department at 602.506.6824 to inform us of the days and times and location for which you will be in operation so that an inspection of the Mobile Food Establishment can be conducted within 10 calendar days from date of this letter. If you do not contact the Department within 10 calendar days, your operating permit will be closed.
