Not approved. The following items need to be corrected before the issuance of a type 3 permit can occur. 1) There is a leak coming from the waste water plumbing under the hand sink. Have leak fixed in a professional manner and demonstrate that the water system can operate without any leaks. 2) The owner was unable to provide a commissary agreement at time of inspection. Provide a commissary agreement that is properly filled out by both parties for all services required. 3) There is no consumer advisory for ceviche and aguas chilies and chinola. Provide an adequate consumer advisory with a disclaimer and reminder for all raw or partially raw food items. Consumer advisory guidance provided. 4) No certified food manager certificate was able to be presented at time of inspection. Provide a Certified food manager certificate for a person that will be on site during operation of the unit. 5) There are small holes in the floor where screws have been removed. Please have holes sealed. No County legal action will result from this inspection. Inspection report printed and handed to the owner.

Verification Visits are inspections of establishments enrolled in the MCESD Cutting Edge Program, which requires an enhanced food safety program and ongoing demonstration of active managerial control.
