Addl Notes: Observed there to be no hand drying paper towels at the handwashing sink located by front counter. PIC supplied paper towels at time of inspection. Hand drying towels must always be available at all hand washing sinks. Residual moisture on the hands can increase the level of touch-contact associated bacterial transfer following handwashing. Corrective Action: Corrected at time of inspection.

Addl Notes: Soda nozzles at customer self-service fountain observed with debris build up on inner surfaces. Please ensure food contact surfaces are cleaned in a manner and frequency that removes and prevents build up of debris and organic matter. Corrective Action: Correct prior to next routine inspection. All food-contact surfaces must be cleaned to sight and touch this must be done once every 24hrs.

Addl Notes: Sliced American cheese observed with an internal temp of 58-65*F while being cold held inside the refrigerated unit at the sandwich make line. The ambient temperature of the unit was 65*F at time of inspection. PIC noticed that the unit was turned off and they turned the unit back on. The ambient temperature of the unit dropped to 38&F before end of inspection. PIC voluntarily discarded the cheese at time of inspection. TCS foods must be cold held at 41*F or below to prevent bacterial growth. Discussed the temperature danger zone (range between 41*F and 135*F) with PIC. Corrective Action: Corrected at time of inspection.

Addl Notes: No time stamp for tomatoes that were being held under time as a control for food safety. Internal temperature of the tomatoes was 53*F at time of inspection. Per PIC, tomatoes were stocked roughly an hour prior to this inspection. Adjusted time stamps were made. Reviewed with PIC and employees that time stamps must always be placed on TCS food items as soon as they are removed from refrigeration and placed on the make line. Corrective Action: Corrected at time of inspection.

Comments:
5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Leaking soda fountain and leak frappe machines to where water is pooling on the floor. The frappe machine is leaking into the refrigerated unit that it sit on. Predefined Comment: Must repair all plumbing to a state of good repair prior to next routine inspection.

6-201.11 - Core: Floors, Walls and Ceilings-Cleanability Addl Notes: Observed a hole on the wall back where equipment like pressure washers is stores, also damaged cove base in the same area. holes along the walk-in cooler doorway. Corrective Action: Correct prior to next routine inspection. Predefined Comment: All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
