Description: Adequate Handwashing Sinks, Properly Supplied and Accessible
Comments:
5-205.11 - Priority Foundation: Using a Handwashing Sink-Operation and Maintenance. Addl Notes: Observed a large bucket in the hand washing sink in the back across from the 3-compartment sick. Discussed with person in charge (PIC) that the handwashing sick can only be used for washing hands to help prevent contamination. PIC removed the bucket at time of inspection. PIC will ensure establishment discontinues use of handwash sink for all activities that are not for handwashing. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-3-302.11 (A1-2) - Priority: Packaged and Unpackaged Food-Separation, Packaging, and Segregation; Protection from Cross Contamination. Addl Notes: Observed a large container of raw chicken wings in the walk-in cooler, near other food and drink items. The container was up against ready-to-serve bottles of beer. Discussed with person in charge (PIC) that the risk of cross contamination is higher in this location. PIC moved the raw chicken to the bottom shelf of the rack at time of inspection. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

3-501.17 - Priority Foundation: Ready-To-Eat Time/Temperature Control for Safety Food, Date Marking. Addl Notes: Observed a large bucket of fresh cut French fries in the walk-in without a date. Also observed two containers of potato salad in the fridge behind the bar without a date. Discussed with person in charge (PIC) that these items need a discard date or a date that it was made. PIC marked foods with a date at time of inspection. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Corrective Action: Corrected at time of inspection.

Addl Notes: Observed that the menu offers burgers to be cooked to consumer preference. The menu has an advisory statement at the bottom but does not have asterisks next to the food options that can be cooked to preference. Offering and serving undercooked proteins requires an approved consumer advisory at all points of sale; and must contain an approved disclosure method and reminder statement. Person in charge (PIC) used a sharpie to put asterisks on each of the menus at time of inspection. Corrective Action: Corrected at time of inspection.

Comments:
7-102.11 - Priority Foundation: Common Name-Working Containers Addl Notes: Observed a yellow mustard condiment container being used to store soap at the hand washing sink. There was no label on the container that would identify it as hand soap and not as mustard. Discussed with person in charge (PIC) that this could be confusing for someone that does not know this is soap. It increases the risk of chemical contamination. PIC wrote on the container "soap" at time of inspection and will replace the container with a container of hand soap by next routine inspection. The condiment container will be discarded after it is replaced and never used to store food. Corrective Action: Corrected at time of inspection.

Description: Plumbing Installed; Proper Backflow Devices

5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed the three-compartment sink in the back of establishment that is leaking into a bagged-lined bucket. The leak starts just before it enters the grease trap. Discussed with person in charge (PIC) that the leak needs to be fixed in order to use the three-compartment sink. PIC will use the other three-compartment sink at the bar until fixed. All plumbing must be maintained in good repair at all times. Re-inspection scheduled for 08/27/24. Corrective Action: Correct within 24 hours.

Description: Physical Facilities Installed, Maintained, and Clean
Comments:
6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed dirt, debris, organic matter, bottle caps, and broken glass under the bar area. Also observed walls with food and grease throughout the establishment. Discussed with person in charge that it is time to clean these surfaces to sight and touch to reduce contamination risk. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
