Addl Notes: Observed the following TCS food items holding 48F in the prep table on the assembly line: cheese, sour cream, chopped lettuce, pico, tamales, chopped cabbage, chopped ham, cooked potatoes, chicken, beef, and rolled beef tacos. PIC stated that the table had been open when serving breakfast. PIC moved the items to the walk-in refrigerator and placed an ice bath on the remaining food items. Also observed a bottle of red salsa and a bottle of green salsa at 61F in the beverage cooler. Another bottle was holding 46F. PIC discarded the salsa holding 61F. PIC stated that customers leave the bottles on their table when dining in. Discussed using 6-hour time as a public health control. Inspector had previously created and emailed a 6-hour time policy to the Operator. The document was emailed to the Operator again. TCS food must be stored at 41F or below when cold holding. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed beef rolled tacos with a discard date of 8/22 kept in the walk-in refrigerator. The tacos were discarded. TCS food items must not be stored longer than 7 days. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed the prep cooler on the assembly line to be holding 48F, captured using inspector's min/max thermometer and after observing the internal thermometer placed in the unit. Inspector lowered the temperature of the unit using the dial. Please ensure refrigeration temperatures remain at 41F or below by the re-inspection date. Corrective Action: Correct prior to re-inspection.

Addl Notes: Observed a large container of red salsa that was covered in the walk-in refrigerator and at 62F. PIC stated that the ingredients were cooked yesterday, and the salsa was blended today. The lid was removed from the container and the salsa was moved into a container that was less than 4 inches deep. Predefined Comment: TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. Corrective Action: Corrected at time of inspection.

3-304.14 - Core: Wiping Cloths, Use Limitation Addl Notes: Observed a wiping cloth bucket with <10ppm chlorine concentration. The bucket was refilled to 50ppm. Soiled wiping cloths must be stored in a chlorine concentration of 50-100ppm. Corrective Action: Corrected at time of inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
