Informed PIC that at least one person who is a Certified Food Protection Manager must be available onsite during all hours of operation. Establishment serves ice cream, an open TCS food, which requires a CFPM. A Certified Food Protection Manager (CFPM) is required to be on the premises during all hours of operation. Corrective Action: Correct prior to next routine inspection.

Addl Notes: Observed a five-gallon spouted container with clear greenish liquid and black pieces of organic matter floating around inside. PIC stated that it is a sugar water mixture for the snow cones that was made about a week and a half prior. Informed PIC that it is adulterated product and needs to be discarded. PIC discarded the mixture at time of inspection. All food must be in good condition, safe, and unadulterated. All adulterated food was discarded at time of inspection. Corrective Action: Corrected at time of inspection.

Description: Food-Contact Surfaces: Cleaned and Sanitized

4-602.11 (E) - Core: Equipment Food - Contact Surfaces and Utensils-Frequency; Non-TCS. Addl Notes: Observed black and yellow organic matter on the inside of the ice machine, on the wall where the door slides open on the top. Informed PIC that the ice machine needs to be cleaned and continue to be cleaned so that it is clean to sight and touch. All food-contact surfaces must be cleaned to sight and touch. Corrective Action: Correct prior to next routine inspection.

Description: Warewashing Facilities, Installed, Maintained, Used, Test Strips

4-301.12 (A-B) - Priority Foundation: Manual Warewashing, Sink Compartment Requirements; 3 Compartment Sink Shall be Provided; Adequately Sized. Addl Notes: Observed three-compartment sink out of order, with metal ice block molds, glass gallon jugs, and five-gallon containers being stored in the three-compartments. Informed PIC that the three-compartment sink needs to be used for ware washing and not storage. PIC removed the items inside the three-comp sink and moved them to a storage space at time of inspection. Observed the faucet of the sink to be missing and covered with a metal cap, allowing no water to enter the sink. The handles of the sink are calcified and cannot move. Informed PIC that the establishment has no way to wash, rinse, and sanitize the food contact surfaces so ice cream serving must be halted until the three-compartment sink is properly functioning. PIC agreed to stop serving ice cream. Three-compartment sink must be set up and installed properly for use. The three-compartment sink must have access to hot and cold water. Please provide a functioning three-compartment sink prior to re-inspection. Corrective Action: Correct prior to re-inspection on 10/14/2024.

Comments:

5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed all the drainpipes in the water filtration room to be sunken into the floor drain next to the water softener. Informed PIC that the drainpipes need to be lifted so that there is an airgap between the outlet of the pipes and the top of the floor drain. All plumbing must be maintained in good repair at all times. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Description: Physical Facilities Installed, Maintained, and Clean

Comments:

6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed an accumulation of debris behind the ice machines in the back room. Observed an abundance of cobwebs, dust, and dirt surrounding the tanks for water filtration and the water softening system. Informed PIC that these areas need to be cleaned and continue to be cleaned at a rate which precludes the accumulation of dirt, debris, and organic matter. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
