5-205.12 (B) - Priority Foundation: Prohibiting a Cross Connection; Non-Drinking Lines Addl Notes: Observed mop sink water faucet with no AVB backflow prevention and a Y-valve cross connection to a chemical dispenser system. Predefined Comment: Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Corrective Action: Correct prior to re-inspection. Re-inspection by 05/09/25.

5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed missing floor sink cover/dome grate in the floor sinks under the three compartment sink and by the ice machine. Observed no air gap at the end of the outlet drain pipe from the mechanical dishwasher. Discussed a 1" air gap is required. Observed prep sink faucet missing water supply handle. Predefined Comment: All plumbing must be maintained in good repair at all times. Corrective Action: Correct prior to next routine inspection.

6-501.18 - Core: Cleaning of Plumbing Fixtures Addl Notes: Observed build up of organic matter on around the flush handle of the urinal in the mens restroom. Observed unsealed and broken edges of the handwash sink. Predefined Comment: All plumbing fixtures (handwash sinks, urinals, and toilets) must be maintained clean. Clean prior to next routine inspection. Maintain fixtures in good repair and easily cleanable. Corrective Action: Correct prior to next routine inspection.

6-201.13 - Core: Floor and Wall Junctures, Coved, and Enclosed or Sealed Addl Notes: Observed separation of tile along the floor wall junction behind the ice machine, near the mechanical dishwasher, by the hand wash sink, and other areas of the kitchen. Observed unsealed floor wall junction throughout the restrooms, under the hand wash sinks, and near toilets. Discussed floor wall junction must be enclosed or sealed. Predefined Comment: Physical facility must be maintained in good repair at all times. Repair physical facility prior to next routine inspection. 6-501.11 - Core: Repairing -Premises, Structures, Attachments, and Fixtures-Methods Addl Notes: Observed holes in the walls by the three compartment sink, mechanical dishwasher, and other areas of the kitchen. Observed gaps in the wall around plumbing lines under the three compartment sink. Predefined Comment: All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. 6-201.11 - Core: Floors, Walls and Ceilings-Cleanability Addl Notes: Observed surface material peeling from ceiling resulting in exposed unsealed surface, above the area between the mechanical dishwasher and the ice machine. 6-101.11 - Core: Surface Characteristics-Indoor Areas Addl Notes: Observed missing tile and exposed unsealed flooring near the back door. Observed unsealed wood used as floor cover next to the back corner of the walk-in cooler. Observed unsealed wood shelving above the stand mixer and adjacent prep table. Observed wall covering in disrepair with cracks and gaps in the wall around the mop sink. Discussed adding splash guard or other waterproof barrier. Predefined Comment: All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Discussed sealing gaps in the ventilation hood panels and along the exterior of the walk in cooler.
Predefined Comment: Upon inspection, a remodel has taken place which may require an application and plans to be submitted, and/or additional permits. Please submit an application, supporting documentation, and fees within 5 business days or legal action may result pursuant to MCEHC Chapter 1, Regulation 4(e). Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home.
Observed an instahot water heater installed under the hand wash sink. Instahot is not connected to a hardwired electrical line and plugged in using an electrical extension cord. Discussed a new construction/remodel permit is required and provided application information.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
