Addl Notes: Observed black and translucent organic matter on the inside components of the ice machine. Informed PIC that the ice machine needs to be treated as a food contact surface and must be clean to sight and touch. All food-contact surfaces must be cleaned to sight and touch. Corrective Action: Correct prior to next routine inspection.

6-501.112 - Core: Removing Dead or Trapped Birds, Insects, Rodents and other Pests Addl Notes: Observed dead roaches underneath the reach-in refrigerator. Informed PIC that the dead pests need to be cleaned up to avoid attraction of other pests and prevent odors. PIC informed that they have pest control services happen every two weeks. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. Corrective Action: Correct prior to next routine inspection.

Comments:
3-304.14 - Core: Wiping Cloths, Use Limitation Addl Notes: Observed a wet wiping cloth sitting on the prep board at a prep station. Informed PIC that wet wiping cloths must be kept in the sanitizer bucket with an approved sanitizer concentration and cannot be left on prep tables. PIC placed the wiping cloth into a sanitizer bucket with an approved concentration at time of inspection. All wet wiping cloths must be stored in sanitizer buckets with approved sanitizer concentrations. Wiping cloths were placed in sanitizer buckets with approved sanitizer concentrations at time of inspection. Corrective Action: Corrected at time of inspection.

Comments:
5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed the black drainpipes from the dish washer and prep sink fallen into the floor drain. Informed PIC that the need to be lifted so that there is a one-inch airgap between the outlet of the pipe and the top of the floor drain. All plumbing must be maintained in good repair at all times. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Description: Physical Facilities Installed, Maintained, and Clean
Comments:
6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed an accumulation of food and other debris underneath the reach-in refrigerator, as well as around and under the bar equipment. Informed PIC that these areas need to be cleaned and continue to be cleaned at a rate which precludes the accumulation of debris. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.
6-201.11 - Core: Floors, Walls and Ceilings-Cleanability Addl Notes: Observed uneven concrete flooring in the kitchen and the bar area, some places holding onto water. Informed PIC that the floor needs to be brought back into good repair so that it is smooth and easily cleanable. Discussed the importance of avoiding porous and uneven surfaces in the kitchen and facilities, to ensure that they are smooth and easily cleanable. Observed blocks of unsealed wood underneath the reach in refrigerator, holding it up. Informed PIC that the wood needs to be replaced with a material that is not porous or absorbent. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

No County legal action will result from this inspection.
Discussed with PIC and bartender that copper cannot be used in contact with carbonated beverages or foods with a pH below 6. Mule mugs with stainless steel on the inside can be used instead. PIC removed the cups from the premises.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
