Observed a 3rd repeat priority foundation violation. No certified food manager was present at the time of inspection or prior routine inspections. Ensure certified food protection manager certification is obtained within 90 days. Ensure certified food protection manager is present when establishment is operating. Addl Notes: Observed dishwasher enter the establishment from the rear entrance and without first washing their hands observed the dishwasher handle clean plates. According to the person in charge the dishwasher was cleaning outside. Predefined Comment: Discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks. Employee properly washed hands at handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: No paper towels observed stocked at the handwashing sink in the ware washing area. Predefined Comment: Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Predefined Comment: PIC provided hand drying provision for handwash sink at time of inspection. Corrective Action: Corrected at time of inspection. Repeat Violation: 3rd.

Addl Notes: In the basin of the handwashing sink in the ware washing area observed a small backpack and box of single use gloves. Ensure handwashing sink is accessible at all times and is not used for purposes other than handwashing. Predefined Comment: PIC relocated all items blocking handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: In the prep refrigerator across from the two door reach in freezer in the kitchen observed a large plate of raw shrimp + cooked crab/48°F and spring mix/49°F. In the glass sushi display closest to the front entrance observed raw salmon/47°, raw Hamachi/46°F, raw tuna/48-50°F, cooked crab/48°F, and cooked eel/50°F. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. After discussion with person in charge it was determined all TCS foods were within safe time parameters (less than 4 hours). Predefined Comment: All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed front glass sushi display used to store TCS foods (see violation #22) maintaining minimum temperature of 47.8°F with inspector's min/max thermometer. According to the person in charge the display doors gaskets need to be resealed/repaired. Discussed servicing the unit. Observed a small amount of ice build on interior of the display. Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below prior to re-inspection. Corrective Action: Correct prior to re-inspection.

4-204.112 (A-D) - Core: Temperature Measuring Devices-Functionality Addl Notes: No thermometers observed in glass sushi display refrigerators used to store assorted cooked and raw seafood. A permanent temperature measuring device is required in any unit storing time/temperature control for safety food because of the potential growth of pathogenic microorganisms should the temperature of the unit exceed Code requirements. In order to facilitate routine monitoring of the unit, the device must be clearly visible. Obtain thermometer to leave inside the glass sushi displays prior to the next routine inspection. Corrective Action: Correct prior to next routine inspection.

Addl Notes: Observed quaternary ammonium sanitizer onsite used to fill sanitizer buckets for wiping cloth that come in contact with food contact surfaces. No test strips for quaternary ammonium sanitizer observed onsite. Predefined Comment: Test strips are required for any sanitizers used with food contact surfaces. Provide sanitizer test strips prior to re-inspection. Corrective Action: Correct prior to re-inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
