Addl Notes: At the bar observed no paper towels stocked at the handwashing sink. Predefined Comment: Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Predefined Comment: PIC provided hand drying provision for handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: In the walk in refrigerator observed a tray of raw chicken stored directly above a tray of raw veal (beef). No overhead contamination observed. Predefined Comment: All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

Addl Notes: In the drawer refrigerator at the entrance of the cook line observed two containers of cooked spaghetti/58°F + 69°F, cooked rigatoni/ 59°F, cooked linguini/ 58°F, cooked chicken/ 55°F, raw shrimp/54°F, and cooked lobster/ 65°F. In the two-door pasta station refrigerator across from the drawer refrigerator at the entrance of the cook line observed cooked spaghetti/ 58°F, cheese ravioli/ 56°F, Bolognese sauce/ 55°F, pesto cream sauce/ 50°F, and buro fuso sauce/58°F. In upper portion of fryer station prep refrigerator observed egg wash mix/54°F double panned. In the upper portion of the pizza station prep cooler observed pizza sauce 51°F, roasted tomatoes/59°F, cooked sausage/ 58°F, and mozzarella cheese/ 48°F. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. After discussion with the person in charge it was determined all TCS foods in the drawer refrigerator and prep cooler had been in the units around 10:30AM. After discussion with the person in charge it was determined the TCS foods in the pizza station refrigerator and upper portion of the fryer prep cooler were within safe time parameters. All TCS foods in the drawer refrigerator and two door pasta prep refrigerators were discarded at time of inspection. All other TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Discussed regularly checking cold holding equipment with probe thermometers with the PIC. Ensure all cold holding equipment is holding at 41°F or less prior to storing TCS foods. Corrective Action: Corrected at time of inspection.

Addl Notes: With inspector's min/max thermometer observed two refrigerators malfunctioning at time of inspection and holding TCS foods above 41°F (See Violation #22).The minimum temperature of the drawer refrigerator was observed at 71.2°F. The minimum temperature of the two door prep cooler across from the drawer cooler was observed at 59.5°F. Predefined Comment: Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below. Service call made at time of inspection by the person in charge. Corrective Action: Correct prior to re-inspection.

4-204.112 (A-D) - Core: Temperature Measuring Devices-Functionality Addl Notes: Exterior thermometer displays on malfunctioning drawer refrigerator and two door pasta station prep refrigerator used to store TCS foods are not functional. Any refrigerator and/or freezer which holds time/temperature control for safety foods shall have a thermometer to monitor refrigerator/freezer ambient temperatures. Thermometer shall be located so it is be readily accessible Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
