For a permanent fix to this violation, check out these resources on our website. https://www.maricopa.gov/5114/Food-Restaurants Addl Notes: No employee with supervisory and management responsibility with the authority to direct and control food preparation that is deemed a certified food protection manager (CFPM) was present at time of inspection. A CFPM is required to be onsite during the hours of operation. According to the establishment's person in charge/ food and beverage manager they have a CFPM, but they were not able to provide a copy at time of inspection. Please obtain certified food protection manager certification and keep certification accessible for health inspections. Corrective Action: Correct prior to next routine inspection. Repeat Violation: 2nd.

Addl Notes: At the handwashing sink near the stove observed utensils (large spoons) stored in the basin of the sink. Handwashing sinks shall be maintained so accessible at all times and not used for purposes other than handwashing. Predefined Comment: PIC will ensure establishment discontinues use of handwash sink for all activities that are not for handwashing. PIC relocated the spoons to the three compartment sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: In the upper section of the open top prep refrigerator next to the burrito press observed cooked chicken/46°F, cooked steak/47°F, roasted corn salsa/46°F, and cilantro cream sauce/46°F. After discussion with the person in charge it was determined all TCS foods were placed in the prep refrigerator around 12PM. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Predefined Comment: All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Discussed using lids and/or closing the upper section of the prep refrigerator when it is not in use. Discussed cold holding logs and monitoring TCS food temperature. Corrective Action: Corrected at time of inspection. Repeat Violation: 2nd.

2-303.11 - Core: Prohibition-Jewelry Addl Notes: Observed food employees preparing food and making smoothies with bracelets on their wrist. Except for a plain ring such as a wedding band, while preparing food, food employees may not wear jewelry including medical information jewelry on their arms and hands. Items of jewelry such as rings, bracelets, and watches may collect soil and the construction of the jewelry may hinder routine cleaning. As a result, the jewelry may act as a reservoir of pathogenic organisms transmissible through food. PIC instructed staff to discontinue wearing bracelets when preparing food at time of inspection. Corrective Action: Corrected at time of inspection.

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed multiple in use knives and spatulas stored in a container of ice water on the counter. Predefined Comment: In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection.

4-501.11 - Core: Good Repair and Proper Adjustment-Equipment Addl Notes: Observed ice buildup near the fans in the back of the small reach in refrigerator next to the front handwashing sink. Predefined Comment: All equipment must be maintained in good repair at all times. Repair all equipment to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

According to the PIC the establishment plans to install a dish machine in the kitchen. At time of inspection, the establishment only has a three-compartment sink in the kitchen (dish machine at coffee bar). Installation of equipment that may affect water demands (i.e. dishwasher/sink installation) will require a remodel application with MCESD. If an NSF approved commercial dishwasher is installed an approved 3-compartment sink is still required.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
