Addl Notes: Observed the wall mounted vegetable cutter with food debris between the blades and other food contact surfaces. Equipment food contact surfaces and utensils shall be clean to sight and touch. The person in charge disassembled the cuttter for warewashing at the time of inspection. Addl Notes: Observed TCS food items placed under time as a control but no written time as a control policy or written time to indicate when TCS food items were removed from temperature control to time control only. TCS food items included roasted tomatoes, cooked pasta sauce, and cooked clam broth. 7-102.11 - Priority Foundation: Common Name-Working Containers Addl Notes: Observed spray bottles at the bar, server station, and by the mechanical dishwasher with no labels. The person in charge stated the spray bottles contained sanitizer solution and degreaser then labeled them accordingly. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. Corrective Action: Corrected at time of inspection.

Comments:
3-304.14 - Core: Wiping Cloths, Use Limitation Addl Notes: Observed wiping cloth stored in sanitizer solution with 0ppm quat sanitizer and wet wiping cloths stored air drying on top of sanitizer spray bottles. The person in charge replaced the sanitizer solution with 150-400ppm quat santizer and stored the air drying wiping cloths in santizer solution at the time of inspection. Predefined Comment: All wet wiping cloths must be stored in sanitizer buckets with approved sanitizer concentrations. Wiping cloths were placed in sanitizer buckets with approved sanitizer concentrations at time of inspection. Corrective Action: Corrected at time of inspection.

Comments:
3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed a knife stored between use wedged between the side of the prep cooler and prep table, tongs stored between use hanging from the handle bars of the oven. Food contact surface of the knife and tongs were in direct contact with non-food contact surfaces of equipment covered with food debris and other organic matter. Predefined Comment: In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection.

6-501.11 - Core: Repairing -Premises, Structures, Attachments, and Fixtures-Methods Addl Notes: Observed holes/gaps on the the walls by mechanical dishwasher and prep sink, broken floor tile with exposed unsealed concrete flooring. Predefined Comment: All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. 6-201.14 - Core: Floor Carpeting, Restrictions and Installation Addl Notes: Observed carpeted floor mats behind the bar next to the three compartment sink and in the back service area by the hand wash sink and ice machine. Discussed restricted use of carpeting in walk in refrigerators, warewashing areas, toilet room areas (including handwashing lavatories, toilets, or urinals), refuse storage rooms, and in food prep areas. Corrective Action: Correct prior to next routine inspection.

Discussed having both hot and cold water supply to the prep sink available upon demand at all times. Discussed caution in preventing the electrical chord of the floor mixer from falling into the floor drain by the prep sink. Discussed maintaining employee drinks on a bottom shelf not over food prep areas to protect food from contamination.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
