Addl Notes: Observed no paper towels at any handwashing sinks in the establishment. Discussed with person in charge to maintain all handwashing sinks adequately stocked at all times. The person in charge replenished the paper towels at all handwashing sinks. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. PIC provided hand drying provision for handwash sink at time of inspection. *2nd Consecutive Priority Foundation Violation.* Corrective Action: Corrected at time of inspection.
5-205.11 - Priority Foundation: Using a Handwashing Sink-Operation and Maintenance. Addl Notes: Observed the handwashing sink in the kitchen area blocked off by a large bin of flour. Observed the handwashing sink in the ice machine area that had beer cans in it. PIC relocated all items blocking handwash sink at time of inspection. PIC will ensure establishment discontinues use of handwash sink for all activities that are not for handwashing. *2nd Consecutive Priority Foundation Violation.* Corrective Action: Corrected at time of inspection.

4-602.11 (E) - Core: Equipment Food - Contact Surfaces and Utensils-Frequency; Non-TCS. Addl Notes: Observed thick, slimy buildup on the soda fountain nozzles. Also obsserved dark, mottled buildup on ice in the ice machine. Discussed with person in charge to clean the soda fountain nozzles and ice machine on a more frequent basis to prevent the accumulation of organic matter inside the nozzles and ice machine. The person in charge will have the nozzles and ice machine cleaned. All food-contact surfaces must be cleaned to sight and touch. Corrective Action: Correct prior to next routine inspection.

Addl Notes: Observed creamer bags, nacho cheese and chili bags all without date marking on them in their respective units. According to the person in charge, the foods in both units had been placed into the units 2 days prior to the start of the inspection. All RTE/TCS foods were properly date marked at the time of inspection. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Corrective Action: Corrected at time of inspection.

Addl Notes: Observed no time stamping on any time as control grab and go foods such as wrapped sandwiches. According to the person in charge, the sandwiches had been cooked approximately 3 hours prior to the start of the inspection. All TCS foods being held under time as a control were properly time stamped at time of inspection. Time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. Corrective Action: Corrected at time of inspection.

3-307.11 - Core: Miscellaneous Sources of Contamination Addl Notes: Observed personal beverages stored above ready to eat foods in the walk-in cooler, such as open packages of hot dogs and pickle buckets. Discussed with person in charge to designate a container on a lower shelf below other foods sold to the public which is for personal beverage storage only and labeled as such. The person in charge relocated the personal beverages below other foods. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed no sanitizer testing strips available upon request at time of inspection. Test strips are required for any sanitizers used with food contact surfaces. Provide sanitizer test strips prior to re-inspection. A re-inspection will occur within 10 days or no later than August 27th, 2026. Corrective Action: Correct prior to re-inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
