Addl Notes: Observed employee peeled raw cucumbers without wearing gloves, per manager, the raw cucumbers will be served without cooking. Discussed the importance of preventing bare hand contact with ready-to-eat (RTE) foods with the person-in-charge (PIC). Bare hand contact is not approved with ready-to-eat (RTE) food items. The cucumbers were placed on embargo at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed no soap dispensing at hand washing sink besides the ware wash machine, plastic container was empty. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Manager provided soap for handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed raw pork and raw beef in open containers stored above picked vegetables, tofu and cooked noodles in open containers in walk-in refrigerator, observed raw pork in open container stored above raw shell eggs in walk-in refrigerator, observed raw pork in open container stored above raw fish in open container. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. Manager properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed cooked noodle, raw bean sprout, tofu, cooked potato, cooked beef, raw octopus, and cooked ham with internal temperature between 54*F to 59*F in prep cooler, the ambient temperature of prep cooler was measured at 47.6*F at time of inspection. Per manager, the above food items were taken out from walk-in refrigerator less than 2 hours prior to inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Manager moved the above food items back to walk-in refrigerator for fast cooling at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed cooked beef, cooked dumpling, cooked pork in the walk-in refrigerator without date marking, manager stated the above food items were prepared 24 hours prior to the inspection. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). All RTE/TCS foods were properly date marked at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed prep cooler in the kitchen with a minimum ambient air temperature of 47.6'F and maintaining all TCS foods between 54*F to 59*F at the time of inspection. Cold holding equipment must have the capacity to maintain all TCS foods at 41'F or below. Must repair prep cooler such that the prep cooler maintains all TCS foods at 41'F or below. Will re-inspect on 09/16/2024. Corrective Action: Correct prior to re-inspection.

3-305.11 - Core: Food Storage - Preventing Contamination from the Premises Addl Notes: Observed raw lamb and raw pork stored on the floor of walk-in refrigerator, observed raw fish in container stored on the floor of walk-in refrigerator, observed raw beef stored on the floor of walk-in freezer. Food must be stored at least 6 inches off the ground at all times to prevent contamination from the premises. Manager put the above food items on shelves at time of inspection. Corrective Action: Corrected at time of inspection.

Description: In-Use Utensils; Properly Stored

Comments:

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed 5 knives stored between prep cooler and prep bench. Food debris was observed on the surface of knives and in between counters. Discussed in-use utensil storage (on clean surface if wash, rinsed and sanitized every 4 hours). Knives were removed and cleaned at time of inspection. In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
