Addl Notes: Observed cook use gloved hands to crack raw shell eggs into a frying pan then use the same gloved hands to handle a clean utensil and clean plates for immediate service. PIC asked cook to wash hands and don a new set of gloves. Employees will be retrained on proper handwashing and when to wash. Contaminated equipment will be washed and sanitized. Discussed washing hands when they become contaminated, such as from raw proteins. Hands should be washed between glove changes, when changing tasks, etc. to prevent contamination of food and food contact surfaces. Corrective Action: Corrected at time of inspection.

Comments:
4-601.11(A) - Priority Foundation: Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils. Addl Notes: Observed at least one pan, turner, the tomato dicer, and the wedger with food debris on the food-contact surfaces and/or blades of the equipment that was stored as clean. PIC placed the soiled equipment into the ware washing area to be rewashed and sanitized. Equipment food contact surfaces and utensils shall be clean to sight and touch. Discussed the hazards of TCS foods left on equipment that is stored at room temperature. Many foodborne illness pathogens grow rapidly in the danger zone (41-135F). TCS food debris remaining on clean equipment is a potential source of contamination for foods that come into contact with the equipment. Corrective Action: Corrected at time of inspection.

Description: Plumbing Installed; Proper Backflow Devices

PRIORITY VIOLATION-5-202.13 - Priority: Backflow Prevention, Air Gap
Addl Notes: Observed a y-valve with 2 attached hoses at the mop sink water spigot (single hose bibb). The mop sink does not have an approved service sink faucet with hot water, cold water, and an approved atmospheric vacuum breaker (AVB) installed. Both hoses observed to be inside of the mop sink, under the flood rim level. PIC removed the hoses and y-valve during the inspection. Provided the backflow preventer handout, an example of an approved faucet, and an example of a hose bibb vacuum breaker which must be installed at the water spigot to protect the water supply from a possible backflow event. Previously, the hose bibb supplied the water to the chemical dispenser and a vacuum breaker was not required. If a hose is attached to the hose bibb then a vacuum breaker is required. Emailed the backflow preventer handout. Discussed installing an approved faucet or the hose bibb vacuum breaker. Corrective Action: Corrected at time of inspection.

Calibrated thermometer. Discussed fish that is in vacuum-packed packaging. Raw frozen fish must be removed from the package prior to thawing.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
