Addl Notes: Observed chicken cutlets on the metal part of the fryer temping at 100F and hashbrowns temping at 100F on a baking sheet above the grill. Cook stated that both food items have been out for 15 minutes. Cook voluntary discarded the hash browns and reheated the cutlets to 165. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. Corrective Action: Corrected at time of inspection.

Comments:

3-302.12 - Core: Food Storage Containers Identified with Common Name of Food Addl Notes: Observed butter in squeeze bottles unlabeled on the cook line and pancake mix unlabeled in the dry storage area in the back. All bulk food items not easily identifiable must be labeled with common name of food. PIC was informed about these containers, was able to identify them and stated that he would label these items. Corrective Action: Correct prior to next routine inspection.

Description: Contamination Prevented during Food Preparation, Storage and Display

Comments:

3-305.11 - Core: Food Storage - Preventing Contamination from the Premises Addl Notes: Observed a large plastic container holding bread covered in a trash bag used as a lid. Discontinue the use of trash bags as a way to store the bread as a lid in the walk in cooler. Fans above the walk in cooler are creating condensation that is leaking onto the bag. Discussed with the PIC about relocation for this item. Observed sealed butter on the floor of the walk in cooler. PIC was made aware and stated he would get someone to pick up the butter. Food must be stored at least 6 inches off the ground at all times to prevent contamination from the premises. Corrective Action: Correct prior to next routine inspection.

Comments:

3-304.14 - Core: Wiping Cloths, Use Limitation Addl Notes: Observed a red wiping cloth bucket underneath a table with plates test at 0ppm sanitizer concentrate. All wet wiping cloths must be stored in sanitizer buckets with approved sanitizer concentrations between 150-400. Wiping cloths were taken to the back by the PIC to be dumped and filled with approved sanitizer concentrations at time of inspection. Corrective Action: Corrected at time of inspection.

Comments:

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed metal tongs over the rails in the grill/stove area. In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. Corrective Action: Correct prior to next routine inspection.

4-501.11 - Core: Good Repair and Proper Adjustment-Equipment Addl Notes: Observed a plastic scooper with a handle chipped and scratched. All equipment must be maintained in good repair at all times. Repair all equipment to a state of good repair prior to next routine inspection. PIC was informed not to use this scooper and find a replacement for this area. Corrective Action: Correct prior to next routine inspection.

Addl Notes: Observed no testing strips by the dish machine, PIC grabbed some from the back but they were expired. Sanitizing using high temperature warewashing machines requires an irreversible measuring device to ensure food contact surfaces reach 160°F. Provide approved unexpired testing strips prior to re-inspection. PIC stated he will order new testing strips prior to re-inspection at 8/28/25 Corrective Action: Correct prior to re-inspection

Description: Plumbing Installed; Proper Backflow Devices

Comments:

5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed a leaky pipe underneath the 3 bay sink with a metal pan underneath to catch the water. PIC stated that he was unaware of what was happening with the pipe. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
