Addl Notes: Observed sushi rice at 77*F the chef stated was leftover from last night in case an order was received before today's sushi rice had finished being prepared. No timestamp observed on the rice. Time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. Food to be cooked and served within 4 hours from time it was removed from temperature control or discarded. Discussed that an acidified rice variance is required if rendering sushi rice shelf-stable. The chef discarded the rice at the time of inspection (see embargo.) A variance/HACCP must be obtained prior to conducting any specialized processes. Variance/HACCP applications are available through the customer portal "Permit Center" at https://maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home. Comments:

7-202.11 - Priority Foundation: Restriction-Presence and Use Addl Notes: Observed 6+ canisters of indoor fogger insecticide in the employee restroom. All pest control for the establishment must be performed by a certified pest control specialist and chemicals for pest control must not be stored onsite. PIC removed pest control chemicals from premises at time of inspection. Corrective Action: Corrected at time of inspection.

Description: Approved Thawing Methods Used

3-501.13 - Core: Thawing Addl Notes: Observed thawed packages of raw shrimp in ROP packaging with a label stating "Keep Frozen" in the prep cooler across from the cook line. All ROP fish labeled "Keep Frozen" must be removed completely from reduced oxygen packaging (ROP) prior to thawing under refrigeration or immediately after thawing under cold running water. All ROP fish was removed from ROP packaging at time of inspection. Corrective Action: Corrected at time of inspection.

Comments:

3-304.14 - Core: Wiping Cloths, Use Limitation Addl Notes: Observed cotton wiping cloths in 0ppm quat sanitizer at the sushi area. Per PIC, the sani bucket was filled up from the dispenser. Observed 150ppm quat from the dispenser. Advised PIC to switch to a wiping cloth of another material that will not bind quat, or to add additional quat within range to allow for proper sanitizing. Advised PIC chlorine sanitizer measuring 50-100ppm may also be used without a quat binding effect. All wet wiping cloths must be stored in sanitizer buckets with approved sanitizer concentrations. Wiping cloths were placed in sanitizer buckets with approved sanitizer concentrations at time of inspection. Corrective Action: Corrected at time of inspection.

Comments:

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed the in-use rice scoop stored in room temp water measuring 86*F. In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection.

Comments:

6-202.14 - Core: Toilet Rooms, Enclosed Addl Notes: Self-closing door to the employee restroom observed to not be functioning. Repair/replace door mechanics to allow for self-closing/self-latching. Corrective Action: Correct prior to next routine inspection.

No County legal action will result from this inspection.
Of note: Observed a leak from the dishwasher AVB at the time of inspection. Dishwasher not in use at the time of inspection. Observed the chemical dishwasher sanitizer concentration at 0ppm after turning on, priming for 3 minutes and running 3 times. PIC stated a work order would be placed on the unit ASAP and would discontinue use of the dishwasher until repairs are completed.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
