Core-2-102.12(A), C: Certified Food Protection Manager (CFPM): Person in charge (PIC) did not have a CFPM certification. Advised that the PIC shall be the Certified Food Protection Manager, and must be present during all hours of operation.

Discussed adequately stocking handwashing sinks with soap and paper towels.
Please continue to work on these items:
1) Provide an approved cove base where missing in the front of the house. There are a few areas where the establishment has top set quarry tile. Provide metal cove base at pizza oven and wall where the establishment has installed a diamond plate finish. No base noted in these areas.
2) Ensure that all finishes, floors, walls, and ceiling, are in good repair and are smooth, non-absorbent, and easy to clean. Re-grout where grout is low to ensure that grout is flushed with flooring. Repair any damaged tiles noted throughout and ensure in good repair. There are block walls in the storage area. If the establishment will be storing food/beverage items along this wall, block fill and provide a level 3 or finish or an approved finish. Please also conduct a deep clean of the wall. Repair any wall damage (note by back exterior door) and gaps in walls noted in storage area
3) Provide a self closer for the restroom door and the back exterior door.
4) Repair the rust noted on walk in cooler door.
5) Provide escutcheon plates where missing and caulk around
6) Provide a split toilet seat
7) Provide adequate ventilation to the restroom. The ventilation has been covered.
8) For the hood, seal hood vent seams with metal colored silicone. Provide missing light under ventilation hood system
