Description: Plumbing and Backflow (FD PR)

Comments:

1) Replace the pull out sprayer faucets at the mop sink and prep sink with rigid, swing faucets to provide an air gap backflow. Provide a hose bib vacuum breaker for any hose bib faucet at the mop sink.

2) Provide 1" minimum air gaps to all indirect drains going to floor sinks, as needed.

3) Plumb the drying shelf unit at the bar to the floor sink and provide a 1" minimum air gap. Ensure rigid piping is utilized.

Comments:

1) Provide a 3/8th inch minimum cove base to floor/wall and millwork/floor junctures at the kitchen, cook line area, restrooms, and bar area, as needed. Recommend rubber cove base. A metal cove base is required at the ventilation hood floor/wall junctures.

2) Refinish all damaged and scuffed wall areas with semi-gloss paint, as needed.

3) Caulk-in all wall holes, plumbing penetrations, and escutcheon plates throughout the facility with silicone. Observed a large wall opening next to the mop sink that needs to be finished.

4) Ensure all bottom shelving pieces at shelving units are provided with a 6" minimum clearance from the floor.

5) Provide splash guards around the prep sink to prevent contamination. The splash guards must match the length of the sink and height of the faucet.

6) Replace the damaged laminate wall boards holding wall shelving units. The laminate countertops are the back bar area are not approved. Please provide a clear epoxy sealer to the countertops.

7) Conduct a deep clean under sink fixtures and bar equipment.

8) Refinish the rusted areas of the walk-in cooler and replace the rusted shelving units in the walk-in cooler.

9) Flash off the gaps where the ventilation hood meets the walls.

10) Resurface or replace the cutting board at the refrigerated make table.

11) Caulk the bar countertop/wall gaps with silicone.

12) Provide a sneeze guard at the expo line to prevent contamination from customers sitting nearby at the bar.

13) Replace the torn refrigerator door gaskets, as needed.

14) Provide a smooth and flush finish around the floor sink located under the warewash machine. Observed open concrete.

15) Provide smooth, washable ceiling tiles to the restrooms and above the mop sink. Observed perforated acoustical ceiling tiles.

16) Provide a clear matte sealer to the wood wall panels in the restroom and fill all gaps between panels with caulking.

Comments/Details

Equipment inspection for a Class 4 10+ E&D permit was conducted with manager Nicole L. without an appointment. The establishment was open and operating at time of inspection. The facility is not approved to operate.

The manager closed the facility at time of inspection. Red sign was posted at time of inspection.

Please correct the following to obtain approval to operate:
1) Provide hot water measuring 120' F minimum to the plumbing system. Observed no hot water at time of inspection.
2) Clear the drain system for the mop sink to ensure it can adequately drain. Observed the drain backing up while running the sink.

An additional major remodel permit will be opened to address the listed items.

At the final construction inspection, ensure hot water measuring 120' F minimum is provided and all refrigerators are on and measuring at 41' F or below.

MCESD inspection report was delivered via email.

Due to the violations noted on this inspection report, County legal action may result from this inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
