For a permanent fix to this violation, check out these resources on our website. https://www.maricopa.gov/5114/Food-Restaurants Addl Notes: No Certified Food Protection Manager was onsite at time of inspection. Informed PIC that at least one person with a Food Protection Manager Certificate needs to be available on site during all hours of operation. PIC stated that they have the certification but were unable to provide evidence at time of inspection. A Certified Food Protection Manager (CFPM) is required to be on the premises during all hours of operation. Corrective Action: Correct prior to next routine inspection.

7-102.11 - Priority Foundation: Common Name-Working Containers Addl Notes: Observed a clear spray bottle with chemical in it and no label. PIC was unsure what chemical was inside the bottle when asked. PIC contacted manager and was informed it is all purpose cleaner. PIC labeled bottle at time of inspection. Corrective Action: Corrected at time of inspection.

Description: Non-Food-Contact Surfaces Clean

Comments:

4-601.11(B) and (C) - Core: Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils Addl Notes: Observed a large accumulation of white, brown, and black residue and debris on the entire bottom of the inside of the reach in cooler at the deepest part of the bar. A bottle of ranch, jar of jalapenos, and case of individual creamers are the only things kept in the cooler. PIC stated that it isn't used anymore except for those things because they cut the menu. Informed PIC that the reach in needs to be cleaned and continue to be cleaned at a rate which precludes the accumulation of dirt, debris, and organic matter. Observed at least a centimeter of water sitting in the bottom of both slide top beer coolers with bottles and the packaging sitting in the water. PIC stated that it fills with water, and they vacuum the water out frequently. Informed PIC that the water needs to be cleaned, and the equipment should be serviced to bring it back into good repair.   Nonfood-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Nonfood-contact surfaces shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Corrective Action: Correct prior to next routine inspection.

6-202.14 - Core: Toilet Rooms, Enclosed Addl Notes: Observed the door to the women's restroom to not be self-closing and the door to the men's propped open with a trashcan. Informed PIC that restroom doors need to be self-closing and not propped open. Please fix the self-closing mechanism prior to next routine inspection. Except where a toilet room is located outside a food establishment and does not open directly into the food establishment such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the premises shall be completely enclosed and provided with a tight fitting and self-closing door. "A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
