Addl Notes: Observed establishment utilize quat-sanitizer for manual warewashing but 0ppm quat-sanitizer in the three compartment sink compartment with sanitizer solution. The sanitizer dispenser was connected to an empty santizer bottle. The person in charge obtained chlorine sanitizer for manual warewashing and placed an order for supply of more quat-sanitizer at the time of inspection. Predefined Comment: Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels, if using quat- sanitizer 200-400ppm and if chlorine 50-100ppm. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed cooked pizza in a display case at ambient temperature under time as a control but no time as a control written policy or time marking. The person in charge stated the pizza was cooked at 1130, less than 3 hours ago, and time marked the pizza accordingly. Predefined Comment: Time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. Corrective Action: Corrected at time of inspection.

7-209.11 - Core: Storage-Other Personal Care Items Addl Notes: Observed employee personal items to include drinks, antiacid tablets, sunglasses/keys stored over the three compartment sink and food prep area. The person in charge removed all the employee personal items and placed them in a designated area away from areas of food prep/storage and clean utensils. Predefined Comment: All employee personal items must be stored away from food, utensils, and single service items to prevent contamination. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed cooked pasta with meat in a container tightly sealed with plastic wrap with internal temperature of 80F in the reach in cooler. The person in charge stated the pasta had been cooked and placed into the reach in cooler for cooling less than 1 hour ago and removed the plastic wraping to continue cooling. Discussed cooling methods to maximize heat transfer. Predefined Comment: TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. Corrective Action: Corrected at time of inspection.

4-204.112 (A-D) - Core: Temperature Measuring Devices-Functionality Addl Notes: Observed various coolers used for cooling and cold holding of TCS foods with no internal ambient thermometer or thermometers that were not readily accessible and easy to read. Any refrigerator or freezer, storing TCS food products, must have a funtioning thermometer in a easily accesible place. The person in charge placed an order for additional theremometers at the time of inspection. Corrective Action: Correct prior to next routine inspection.

6-101.11 - Core: Surface Characteristics-Indoor Areas Addl Notes: Observed missing cove base under the three compartment sink and ceiling tiles in disrepair over areas of food and food equipment storage. Discussed installing cove base in areas of high moisture and replacing ceiling tiles to be smooth and easily cleanable. Predefined Comment: All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
