1. The hand washing sink in the front area has a broken hot water valve and very low and inadequate hot water pressure. Repair the faucet on the front hand sink so that the hot and cold water valves are fully functional.

PRIORITY VIOLATION-5-203.15, P: Backflow Prevention Device. Carbonator;
1. There is no backflow prevention device installed for the water supply line to the soda fountain carbonators. Provide a certified, RPZ backflow prevention device for the water supply line to the soda fountain carbonators. All piping and fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass. The device must be tested by a certified backflow testing company after installation.

1. There are two hose bibs underneath the hand sink in the back kitchen area that are leaking into a bucket. Provide hose bib vacuum breakers for each of the hose bibs and repair all leaks.

1. The water heater is leaking and this is resulting in standing water on the floor of the mop sink room. Repair all leaks in the water heater. If the water heater is replaced, a minimum 50-gallon capacity water heater is required.

PRIORITY VIOLATION-5-402.11, P: Backflow Prevention;
1. The three compartment sink is directly connected to the sewer system and is the only sink available for washing vegetables and thawing foods. An additional, NSF approved food prep sink that has hot and cold water and an indirect drain pipe that empties into a floor sink must be installed. There must be a minimum 1 in. air-gap between the end of the drain pipe from this sink and the flood rim of the floor sink that it drains into.
2. The soda fountain nearest to the drive through window has a drain pipe from the ice bin that connects to the soda waste pipe. This pipe runs into the floor then up out of the floor before emptying into a floor sink next to the ice machine. This soda fountain must be relocated so that the ice bin and soda waste can drain properly and indirectly into the floor sink with separate drain pipes from the ice bin and from the waste.
3. The salsa bar drain pipe is connected directly to the drain pipe from the self-service soda fountain waste pipe. There must be a minimum 1 in. air-gap between the drain pipe from the salsa bar and any connections to the drain pipe for the soda fountain waste.

1. The drain pipe for the three compartment sink leaks onto the floor from several spots. Repair all leaks in the three compartment sink drain pipe.

This establishment is not participating in the award program at this time and had 5 Priority and 1 Priority Foundation violations on this inspection.

This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
