Handwashing. __ Noted the food prep sink is located in the walk-in cooler's room. The

nearest hand sink is in the kitchen/ ware washing area and not visible in this work zone.

Provide an additional hand sink in the food prep area.

only. A dump sink/ pre-rinse sink can be provided for this purpose. Owner removed the

soiled cups from the hand sink.

Methods/Existence __ Noted rodent droppings and fruit flies inside the bar, soda's bag & box

rack, water heater's areas. Per owner, a pest control company had been contacted and

evaluated the issue. Please provide pests control plan and monitoring log. Pests must be

eliminated in order to obtain a permit to operate.

Core-6-202.15, C: Outer Openings, Protected ___ Noted gaps under and side of exterior doors

and the air curtain at the delivery door is control by breaker switch and not by the micro-

switch at the door anymore. Please provide door sweeps, weather stripping where missing at

exterior doors, and repair the micro-switch of the air curtain at the delivery door. The

air curtain also have rattling noise. Please repair.

handles, floor sinks, and equipment doors are soiled with accumulation of organic matter.

Please clean frequently to prevent accumulation and maintain clean.

in freezer are missing plugs on the walls and ceiling panels. There are gaps at walls and

hood panels. Please seal all missing plug (or provide new plugs) and gaps with wide range

___ Noted there is a new 50-gallon water heater in the facility. The establishment has a

dish washer. Please provide a 100-gallon tank water heater with 100% recovery rate or

install a tankless water heater upstream of the existing tank water heater to meet the

demand. Provide a 3 gallon per minute tankless water heater at 60 degree rise if a tankless

water heater will be used in-tandem with the existing tank water heater. See hot water

calculation sheet.

PRIORITY VIOLATION-5-202.13, P: Backflow Prevention, Air Gap__ Noted the drainpipes for the

drip trays of the keggerators are discharging below the flood rim of the floor sink.

Provide a hub/ funnel drain for these equipment to discharge into. Provided pictures of hub

drain and riser.

Priority Foundation-5-205.13, Pf: Scheduling Inspection and Service for a Water System

Device __ Noted the water filter of the ice machine was last replaced in 2018. The backflow

preventer for the soda carbonator and dishwashing machine had not been service/ replace and

there are signs of leaking on the units. Replace/ repair the atmospheric vacuum breaker of

the dishwasher, and test / repair the RPZ of the soda carbonator and provide reports.

Comments:

Core-6-201.11, C: Floors, Walls and Ceilings-Cleanability __ Noted the floor in the bar is

missing grout between tiles, areas around the floor sink in the bar is at aggregate level,

there is a hole at the floor sink of the bar's 3-comp sink. Fill missing grout. Grout

and floor must be at the same level. Concrete filler must be smooth and non-absorbent.

Provide several coats of marine grade sealant on the brick walls inside the bar. Provide

FRP boards behind the wet wall of the bar (where the 3-comp sink and the jockey boxes

are). Provide several coats of semi-gloss paint on the substrate of the countertop.

Core-6-201.12, C: Floors, Walls, and Ceilings, Utility Lines__ Please provide missing

cleanout covers on walls where missing. Remove old data lines and ensure all electrical /

data wiring must be enclosed inside conduits.

Core-6-201.13, C: Floor and Wall Junctures, Coved, and Enclosed or Sealed__ Walls and

floor junctures in the bar and adjacent storage, water heater room is not provided with

the 3/8" radius cove base. Please remove the straight baseboards and replace with the

approved type.

Core-6-201.14, C: Floor Carpeting, Restrictions and Installation __ The upstairs storage

room floor is carpeted. Please remove the carpet and provide smooth, durable, non-

absorbent floor surface material.

Core-6-201.15, C: Floor Covering, Mats and Duckboards __ Noted the floor mats inside the

bar has heavy accumulations of soil/ dirt. Please clean frequently to prevent

accumulation.

Core-6-501.114, C: Maintaining Premises, Unnecessary Items and Litter

__ Noted numerous fixtures, equipment, wares that owner indicated not being used inside

the storage room, inside the food prep sink, kitchen and mop sink's area. Please remove

all un-use equipment, fixtures, tools, or materials off the premises.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
