1. Food employees were observed thawing fresh packages of food in the large scrap/rinse sink that is missing a front leg next to the three compartment sink. All food items were removed from this sink during the inspection.

1. The cutting boards on the pizza prep cooler in the back kitchen and the cutting boards on the main cook line prep coolers are deeply scored with food debris inside the gouges that cannot be removed. Replace or resurface these cutting boards to be smooth and easily cleanable.
2. The plastic deflector shield in the main ice machine is cracked and broken in several areas and it is hanging on by only one screw. This broken plastic deflector shield inside the ice machine must be replaced.

PRIORITY VIOLATION-5-203.14, P: Backflow Prevention Device, When Required;
1. There is no backflow protection provided on the water supply line feeding the patio mister system. Provide a certified, drinking water safe, RPZ backflow prevention device for the water supply line to the patio mister system.
2. There is an unused pot filler behind the equipment underneath the back hood that is leaking and that has a shutoff valve connected downstream from an atmospheric vacuum breaker. Remove the unused pot filler on the white hose and atmospheric vacuum breaker and properly cap off the water supply line since it is not being used.
PRIORITY VIOLATION-5-205.15 (A), P: System Maintained in Good Repair; According to Law;
1. There is no hot water available at the cook line prep sink. Repair the faucet on this sink so that the hot and cold water valves are fully functional
2. The hot water does not shut off on the right faucet of the three compartment sink. Repair the faucet on this sink so that the hot and cold water valves are fully functional.
3. The large rinse/dump sink adjacent to the three compartment sink is missing a front support leg and it is detaching from the wall. Repair this sink to be properly supported and attached to the wall.
4. There is a flex water line draining water from the ceiling above the three compartment sink that extends down into the floor sink beneath the three compartment sink. Remove this flex water drain line or provide a hard (PVC type) drain pipe that drains into the floor sink with a minimum 1 in. air-gap.
5. The beer tap drain pipe in the bar is a flex pipe that does not drain properly into the floor sink beneath the beer tap. Provide a hard (PVC type) drain pipe for the bar beer tap.
Priority Foundation-5-205.13, Pf: Scheduling Inspection and Service for a Water System Device;
1. The RPZ backflow prevention devices for the soda carbonator and for the coffee/tea machines must each be tested and certified.

1. There is a buildup of garbage, grease, soil, food debris and wet rags underneath every sink, piece of equipment and cabinet throughout the kitchen, front pizza prep area and bar. Thoroughly clean everything and remove all trash.
Core-6-101.11, C: Surface Characteristics-Indoor Areas;
1. Repair the damaged walls, floor tile and missing grout beneath the mechanical dishwasher and beneath the three compartment sink.
2. Remove the duct tape from the cleanout cover beneath the mop sink faucet.
3. Replace all damaged, missing and non-smooth ceiling tiles in the kitchen and water heater room with ceiling tiles that are smooth, non-absorbent and easily cleanable.

Re-inspection is scheduled for 02/01/2024.
This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable.
On July 1st, 2023, the Environmental Services Department started emailing permit card(s) and permit invoice(s) to permittees. We need a current and accurate email address on file to ensure you receive a copy of your permit card and invoice(s) upon renewal.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
