PRIORITY VIOLATION-5-101.11, P: Approved System-Source. Ice and water for the coolers on the property are sourced from the maintenance shed located along McDowell Road. This will be changed and the water and ice will be obtained from the auxiliary structure with three compartment sink and hand sink. Make sure all fixtures in that room are in good working order and stock with soap and paper towels. Manager is purchasing a food grade water hose today.

a). The drain line on the dish washer will need to be lifted and end above the floor sink

b). Please extend the ice bin drain and raise the drains on the ice machine at waiter station.

c). Mount the water filter of the floor, unit is upstream of the soda system.

d). At the bar, make the floor sink under the keg cooler accessible for cleaning, notch the toe kick in if needed. Extend the drip tray drain so it clears the toe kick of the keg cooler.

e). Establishment does not have a food prep sink, please install with hot and cold water and provide a indirect waste connection trough a hub drain or lead to the floor sink.

f). The three compartment's third valve is frozen, please repair.

g). Please remove old caulk and re-caulk the three compartment sink and dish gutter at the wall junctures.

h). Raise the drain line of the ice bin and extend and mount the soda gun holster drain line to this drain.

a). The dual check vale on the carbonator will need to be replaced or checked by the soda company and tagged with a tag stating the dual check valve on the carbonator is in good working order.

b). For all old and new non-carbonated beverage machines, install one if you split the lines, or multiple where needed dual check valves that meet plumbing standard ASSE 1022 on the water supply lines. If you do not install a filter provide debris screens before the dual check valves or install after the filter before the units.

Establishment is planning the following in the next 30 days:
1). New dishwasher at the bar, make sure to transition flex line into a rigid drain line and lead to the floor sink. Mount along the wall and then forward. Maintain air gap.
2). Dishwasher in the kitchen will be replaced.
3). New beverage equipment will be installed. See note about dual check valve.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
