Comments/Details

OC inspection conducted with owner Danielle T. Establishment is not selling any TCS or opened food/drinks to the public. Only unopened non-TCS foods/drinks are being sold to the public. Establishment is in the process of obtaining a new dishwasher, walk-in cooler, new soda system and new equipment for the hood. Hood will require a sign off from the fire marshal/city of Mesa.

***A remodel permit will be created. Owners will need to provide cut sheets for the dish washer and walk-in cooler.
***A second remodel will be required for the addition of a new bar. Discussed needing plans and cutsheets for the bar. Bar will be the second phase once the kitchen is approved.

Correct the following items prior to final inspection:

1) Provide an NSF approved prep sink connected to hot and cold water. Provide a rigid drain that drains to the floor sink. Provide a 1-inch air gap above the floor sink. Do not tie drain line into the hand sink drain lines.
2) Provide two additional NSF approved hand sinks. One in the ware wash area, and one on the other side of the wall next to the prep sink. Provide splash guards to the hand sinks if within 24-inches of prep sink or food items/equipment.
3) Current hood is a type 2 hood. Contact the fire marshall/city of Mesa for hood requirements and provide the approval. A type 1 hood is required for grease laden foods.
4) Provide hot water to all sink fixtures. Hot water must reach all sink fixtures within 45 seconds. All hand sinks must have a hot water temperature of 100-110’F and all other sinks must have a hot water temperature of 110’F or above.
5) Provide/relocate RPZ (ASSE 1013) backflow device for the soda system once installed. Ensure there is no copper or brass lines/fittings downstream of the RPZ and soda equipment. Test and certify the backflow device and provide the test report.
6) Ensure each chemical dispenser has an internal air gap (ASSE 1055A).
7) Provide a consumer advisory statement with asterisks next to any undercooked hamburger items.
8) Replace any burnt out lights throughout the establishment. Provide any missing light covers. All light covers must be shielded.
9) Adjust the flow rate for the 3-compartment sink.
10) Provide cut sheets for the walk-in cooler and dishwasher.
11) Provide any cleanout covers/escutcheon plates where missing.
12) Ensure walk-in cooler has a 3/8” stainless steel cove base inside and outside of the walk-in.
13) Flash the sides of the walk-in cooler.
14) Provide dry wall, FRP, or stainless steel to any exposed CMU.
15) Repair air curtain as needed. May also install self-closing doors for both restrooms.

The Environmental Services Department will begin emailing permit card(s) and permit invoice(s) to permittees in the coming months. We need a current and accurate email address on file to ensure you receive a copy of your permit card and invoice(s) upon renewal.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
