PRIORITY VIOLATION-3-302.11 (A1-2) - Priority: Packaged and Unpackaged Food-Separation, Packaging, and Segregation; Protection from Cross Contamination. Addl Notes: Observed multiple raw shell egg flats stored above edamame, steamed broccoli, and shredded cabbage in the bottom of a prep unit. Informed PIC that raw animal protein must be stored below an RTE foods. PIC stated that the eggs are for the employees and not for the food in the establishment. Informed PIC that employee foods must be stored in a manner that does not risk contamination of food meant for the consumers. PIC properly stored the raw shells eggs and RTE foods at time of inspection. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

4-602.11 (E) - Core: Equipment Food - Contact Surfaces and Utensils-Frequency; Non-TCS. Addl Notes: Observed a large accumulation of translucent and brown organic matter on the inside lip of the ice machine located in the back storage room. Informed PIC that the ice machine needs to be cleaned and continue to be cleaned at a rate which precludes the growth of organic matter. All food-contact surfaces must be cleaned to sight and touch. Corrective Action: Correct prior to next routine inspection.

Addl Notes: Observed a bin of cream cheese wontons stored on the bottom shelf of a prep table with an internal temperature of 82*F. Employee stated that the wontons were cooked thirty minutes prior to inspection. Informed PIC that the wontons cannot be left to sit at room temperature. Discussed with PIC that the wontons must be held at 135*F or above for proper hot holding. Discussed options with PIC including cold holding the wontons and cooking them to order or placing these items on time as a control (documentation needed). PIC placed the wontons into the walk-in refrigerator at time of inspection to cool down to 41*F. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed containers of cooked shrimp, steamed broccoli, and cooked noodles with internal temperatures ranging 56*F-82*F cooling in the walk-in and bottom of a prep unit with plastic rap covering the items. Informed PIC that items cooling need to remain uncovered until the product is cooled down fully. PIC uncovered the items cooling at time of inspection. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. All TCS foods cooling were uncovered to allow for rapid cooling. Corrective Action: Corrected at time of inspection. ***** 2nd Consecutive Violation *****

5-403.12 - Core: Other Liquid Wastes and Rainwater Addl Notes: Observed the condensate line for the shaved ice machine draining into the three-compartment sink behind the sushi station. Informed PIC that the condensate line needs to drain into a floor drain. Corrective Action: Correct prior to next routine inspection.

1. Discussed proper storage of in-use utensils with PIC. 2. Discussed that unsealed wood should not be used in the construction or repair of equipment in kitchen areas. 3. Discussed ice for display on customer plates needs to be disposed of and tableware washed, rinsed, and sanitized after every customer.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
