1. An employee was observed drinking out of an open cup while prepping foods in the main prep area. The employee was instructed to remove the open drink and wash hands before returning to food prep.

1. There is no hand sink provided in the mechanical dishwasher room. Provide a dedicated hand sink with hot and cold water, a separate drain pipe and side splashed in the mechanical dishwasher room or remove the mechanical dishwasher.

1. There is an old RO system installed at the hand sink in three compartment sink room without backflow protection. If this currently inoperable, RO system remains a certified, RPZ backflow prevention device must be installed upstream from the system and all piping and fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass. Also, the faucet must be removed from the hand sink area. Removing the RO system entirely is an acceptable correction.
PRIORITY VIOLATION-5-202.13, P: Backflow Prevention, Air Gap;
1. There is a hose connected to the utility sink faucet in the three compartment sink room that extends into the sink. Provide a new, hose bib vacuum breaker for this faucet or remove this sink.

1. The drain pipe for the hand sink in the three compartment sink room extends into the floor sink that it drains into. Provide a minimum 1 in. air-gap between the end of this pipe and the flood rim of the floor sink. This same correction must also be applied to the drain pipe for the food prep sink that extends into the floor sink adjacent to the ice machine.
2. Replace all missing strainers and grates for all floor sinks.

The following items must be corrected prior to re-inspection;
1. There is deteriorated and soiled carpet installed in the dishwasher area, back mop sink room and food and utensil storage room. All carpeting must be removed from these room and the floors must be finished to be smooth, non-absorbent and easily cleanable.
2. There are popcorn ceilings installed above the dishwasher, in the back utensil storage room and in the dry goods storage room. The ceilings in all these areas must be finished to be smooth, non-absorbent and easily cleanable.
3. The walls underneath the three compartment sink and around the adjacent utility sink are deteriorated with peeling and missing cove base. Provide a waterproof finish such as FRP to these walls and provide new cove base finish at the floor/wall junctures in these areas.
4. Repair the rough floor around the grease trap lid below the three compartment sink and the pit in the floor in the walk-way between the hood an walk-in unit to be smooth, non-absorbent and easily cleanable.

1. There is missing or deteriorated cove base in the back mop sink room, all restrooms, and the dry goods storage room. Provide a cove base finish at all floor/wall junctures in these areas and anywhere else that food and utensils are stored.

This establishment is not participating in the award program at this time and had 3 Priority and 1 Priority Foundation violations on this inspection.
If violations are not corrected by established timeframes, legal action may result.
Re-inspection is scheduled for 02/27/2024.
This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable.
On July 1st, 2023, the Environmental Services Department started emailing permit card(s) and permit invoice(s) to permittees. We need a current and accurate email address on file to ensure you receive a copy of your permit card and invoice(s) upon renewal.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
