Addl Notes: Observed employee chopping lettuce without gloves. The lettuce was placed in a container and put directly into the prep table. Discussed with PIC the importance of preventing bare hand contact with ready-to-eat (RTE) foods with the person-in-charge (PIC). Bare hand contact is not approved with ready-to-eat (RTE) food items. RTE food that was contacted by bare hands was discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed at the hand washing sink at the front of the restaurant below the menu to not be supplied with paper towel or soap. Discussed with PIC handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. PIC provided paper towels and soap for handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed on the white apron of the ice machine to have a light brown and dark black slimy organic matter. Discussed with PIC the all food-contact surfaces must be cleaned to sight and touch. Corrective Action: Correct prior to next routine inspection.

3-501.15 (A) - Priority Foundation: Cooling Methods. Addl Notes: Observed a container of chopped lettuce and a container of chopped lettuce with tomato with an internal temperature of 58*F in the top of the prep table. Per PIC the 2 items were prepped and placed in the unit 30 minutes prior to inspection. Discussed with PIC TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means and cooling shall not take place in the top or the bottom of the prep table. PIC relocated the chopped lettuce and the chopped lettuce with tomato to rapidly cool. Corrective Action: Corrected at time of inspection.

Comments:

4-302.12 - Priority Foundation: Food Temperature Measuring Devices Addl Notes: Observed facility to not have a probing thermometer available. Per PIC there is one but was not able to find it. Discussed with PIC properly calibrated probe thermometers are required at all times for monitoring food temperatures. Must provide a probe thermometer for establishment prior to re-inspection. Corrective Action: Correct prior to re-inspection. Reinspection scheduled on 10-23-24

Description: Plumbing Installed; Proper Backflow Devices

Comments:

5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed at the hand wash sink in the back of the restaurant to slowly drain and to leak from the twist handle waste valve. Discussed with PIC to have the hand wash sink fixed/ repaired/replaced to properly drain and to stop leaking. All plumbing must be maintained in good repair at all times. Corrective Action: Correct prior to next routine inspection.

No County legal action will result from this inspection.
Of Note: Discussed with PIC to hot holding items shall be heated to 135*F and above before placing in hot holding equipment. Also discussed to not reuse single use containers as they are not designed to be washed and reused. When storing scoops in foods the handle shall be stored in the upright position.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
