Addl Notes: 1. The inner surface of the glass door inside the microwave oven is shattered with sharp exposed shards of glass protruding from the surface. This microwave oven can no longer be used in its current condition at this location or any other location. Corrective Action: Correct prior to re-inspection.

PRIORITY VIOLATION-5-205.15 (A) - Priority: System Maintained in Good Repair; According to Law
Addl Notes: 1. The caulking around the side splash of the hand sink connected to the three compartment sink is severely deteriorated and prevents proper cleaning of the surfaces on both sides of the side splash including the drainboard of the three compartment sink. Remove all the old and soiled caulk and reseal the edges of this side splash with metallic silicone caulk. 2. The drain pipe for the hand sink at the three compartment sink drains partially onto the floor instead of into the floor sink. Reposition the drain pipe for this hand sink so that it empties completely into the floor sink without causing water to collect on the floor beneath the three compartment sink. 3. Condensate from the walk-in freezer compressor is leaking onto the food shelves inside the unit and the drain pipe on the outside of the unit is installed to drain directly onto the floor instead of into the nearby floor sink. Repair the condensate leak inside the walk-in freezer and extend the condensate drain pipe on the outside of the unit to drain into the floor sink with a minimum 1 in. air-gap between the end of the drain pipe and the flood rim of the floor sink. 4. The hot and cold water valves on the food prep sink and on the women's restroom hand sink are reversed. Provide hot water on the left valves and cold water on the right valves for both of these sinks. 5. The drain pipe for the ice machine on top of the soda fountain is a flex drain line that extends into a makeshift electrical and duct tape end piece. Provide a new, separate hard pipe (PVC type) drain pipe for the ice machine at the soda fountain. This new, separate drain pipe must empty into the floor sink with a minimum 1 in. air-gap between the end of the pipe and the flood rim of the floor sink. Corrective Action: Correct prior to re-inspection.

The RPZ backflow prevention device for the soda fountain carbonator must be tested and certified. 2. Provide an ASSE1022 backflow prevention device for the water supply line to the bread oven/proofer. Corrective Action: Correct prior to re-inspection.

Addl Notes: 1. The drain hub assembly beneath the left compartment of the three compartment sink is severely deteriorated and leaking and there is also a leak in the P-trap area of this drain pipe. Replace the drain hub assembly beneath the left compartment of the three compartment sink and ensure there are no more leaks anywhere beneath this sink. 2. The drain valve beneath the middle compartment of the three compartment sink is broken and unable to hold water in the basin so that it can be filled. Repair the drain valve beneath the middle compartment of the three compartment sink so that it can be properly filled. Corrective Action: Correct prior to re-inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
