Methods/Existence __ Noted roaches presence inside the kitchen, under the wok station.

Please contact pest control company for service to eliminate the pests. Owner showed pest

control record and informed that pest control service twice a month. Noted grease built up

on equipment, floor, walls and wok station. Instructed owner to clean up grease built up,

take out trash and clean catch trays under the wok station daily to remove the food source

for the pests. Close / seal all gaps around pipes/ conduits penetrations, holes, cleanouts,

trims, etc. on walls and ceiling. Put ceiling tiles back in place.

OC inspection was conducted with the owners, Bill Zou and partner. The establishment is open and operating.

Permit is not approved.

Please address the following items prior to the re-inspection:

1. Contact pest control for service, clean up the facility first prior to service as discussed. Pests must be eliminated in order to permit to be approved.
2. Provide floor sink for the new steam table. Having steam table discharging via hoses across the floor into the nearest floor sink is not acceptable. Other option: obtain a steam table without drain, provide sump pump that can evacuate the discharge liquid over the ceiling grid into the floor sink across the floor 3 feet away, or relocate the steam table to the wall that has the floor sink.
3. Repair/ replace the cold-water valve at the second faucet of the 3-comp sink. It is leaking.
4. Per owner, the hand sink at the front food service counter will be replaced with another one, new countertop will be provided for this area also.
5. Noted lights under the hood is burnt. Replace all dimmed/ burnt light in the kitchen. Food prep area must have at least 50-foot candlelight (day-light).
6. Re-organize and only keep what is necessary for everyday use inside the facility. Minimize cardboard boxes storage to remove hiding places for the pests.
7. Provide new door sweep/ weather stripping at exterior doors to prevent entrance of pests.
8. Relocate the first aid product to lower shelving. Not above ice machine, clean wares, or dry goods.
9. Avoid using cardboards. Noted cardboard was used to fabricated as a wok’s switch’s shield and the back side of the cardboard has roach legs on it.
10. Detail clean the facility: inside/ outside of cooking equipment, reach-in refrigerators and freezer. Fryers, splashguards, walls behind the hood, under the wok station, floor sinks, strip curtains, walk-in’s vent covers, doors and their handles. All food/ non-food contact surfaces must clean to sight and touch.

Due to violations noted, County legal action may result from this inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
