1. The cold water valves on the hand sink nearest to the dishwasher area and on the hand sink adjacent to the conveyor oven are both broken. Repair the faucets on both of these hand sinks so the cold and hot water valves are fully functional.

2. The drain pipe for the hand sink in the outside patio area is disconnected and laying on the ground causing the hand sink to drain onto the cement. Repair the drain pipe for this hand sink so that it drains properly into the nearby floor sink with a minimum 1 in. air-gap between the end of the drain pipe and the flood rim of the floor sink.

1. Measured the temperature of salsa being held at 47°F-50°F in the prep cooler next to the soda fountain in the main kitchen area. The compressor inside this unit was covered in solid ice. Repair this perp cooler to maintain foods at 41°F or less.

1. There is condensate leaking onto the floor from the glass cooler unit in the bar area. Repair the condensate leak from this cooler unit..

2. A home style refrigerator is being used for whipped cream and other food items next to the dipper well near the main ice machine. Only commercial grade equipment is permitted for TCS food items. Remove the small home style refrigerator from the kitchen.

3. Repair the broken metal door and broken ice bin rim on the soda fountain adjacent to the salsa cooler in the front kitchen area.

1. Provide a Certified, drinking water safe, RPZ backflow prevention device for the water supply line to the booster pump that feeds the soda fountain carbonators. There is currently a double check valve assembly installed instead of an RPZ Backflow prevention device. All piping ad fittings on the downstream side of this device must be plastic or stainless steel as is currently the case. Provide a funnel drain assemble beneath this device that drains to the nearby floor sink with a minimum 1 in. air-gap between the end of the drain pipe and the flood rim of the floor sink. This device must be tested and certified after installation.

2. A certified RPZ backflow prevention device must be installed for the patio mister system or the mister system pump must be disconnected and removed. This device must be tested and certified in the mister system remains.

PRIORITY VIOLATION-5-205.15 (A), P: System Maintained in Good Repair; According to Law;

1. There is no drain pipe for the ice bin at the east end of the bar and water is draining onto the floor. Provide a hard pipe drain pipe such as PVC for this ice bin . This drain pipe must be new and separate and must not connect to any other drain pipes. This drain pipe must also drain indirectly to the nearby floor sink with a minimum 1 in. air-gap between the end of the drain pipe and the flood rim of the floor sink.

2. There is a flex drain pipe for the hand sink at the east end of the bar and a garden hose for the dipper well drain pipe in the ice machine area. Provide hard pipe drain lines such as PVC for these fixtures that drain to floor sinks with minimum 1 in. air-gaps between the ends of the drain pipes and the flood rims of the floor sinks.

1. The drain pipe for the three compartment sink is leaking onto the floor beneath the sink. Repair all leaks in the three compartment sink drain pipe.

2. The drain pipe for the scupper drain next to the mechanical dishwasher is leaking onto the floor. Repair all drain pipe leaks in this fixture next to the opening for trays to be placed and run through the mechanical dishwasher.

3. The drain pipe for the two-compartment food prep sink is leaking. Repair all leaks in the two compartment food prep sink drain pipe.

This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. "A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
