Establishment was open and operating. A Certified Food Protection Manager (CFPM) is required to be on the premises during all hours of operation. PRIORITY VIOLATION-4-501.114 (A-E) - Priority: Manual and Mechanical Warewashing Equipment, Chemical Sanitization- Temperature, pH, Concentration and Hardness. Addl Notes: Observed employee washing dishes in the low temperature warewashing machine; observed 0 ppm chlorine on the dishes. The machine was primed continuously, no chlorine was observed being dispensed despite the chlorine concentrate not being depleted. Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels. The 3-compartment sink was set up with a chlorine sanitizer with 100 ppm chlorine and the employee sanitized dishes in the 3-compartment sink. Person in charge indicated they would call for service for the warewashing machine. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed refried beans, chorizo, and broth with internal temperatures between 95*F - 98*F in the steam table next to the flat top grills. The water in the water bath had an internal temperature of 99*F and the steam table was observed off. Person in charge indicated these foods were prepared ~1 hours before inspection. All TCS (time/temperature controlled for safety) foods improperly hot held were reheated to 165°F at time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed roasted tomatillos and queso fresco labelled 11/25. Person in charge indicated these dates were incorrect and they were actually prepared 11/30. Person in charge indicated these containers were washed but the old label was not removed, and a new label was not added. RTE (ready to eat)/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). All RTE/TCS foods were properly date marked at the time of inspection. Corrective Action: Corrected at time of inspection.

4-501.11 - Core: Good Repair and Proper Adjustment-Equipment Addl Notes: Observed a buildup of ice in the walk-in freezer covering the floors and walls; the unit is also making a hissing sound. Also observed refrigerated prep unit closest to the dry storage closet with gaskets in disrepair. All equipment must be maintained in good repair at all times. Repair all equipment to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. REPEAT VIOLATION (2ND CONSECUTIVE)
4-101.11 (B-E) - Core: Characteristics- Multiuse; Durable, Nonabsorbent, Smooth, Easily Cleanable Addl Notes: Establishment is using old sour cream containers to store restaurant foods. These containers were not designed to be warewashed continuously and maybe inadequately cleaned or break easily. Discussed removing all former retail food containers used for storage of restaurant foods and replacing them with approved multiuse food safe food storage containers. Person in charge indicated they would begin to phase these containers outs. Corrective Action: Correct prior to next routine inspection.

Observed menu with consumer advisory. The consumer advisory includes that the foods are raw/undercooked, there are asterisks on the foods it applies to, but it is missing the reminder that these foods may cause illness. Person in charge indicated they are printing new menus in January and will update the advisory to include this verbiage. See example verbiage below: These foods contain ingredients that maybe raw/undercooked. Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness, especially if you have certain medical conditions."
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
